【摘要】 目的:探讨参附注射液治疗慢性阻塞性肺疾病（COPD）合并Ⅱ型呼吸衰竭效果及安全性。方法:选择本院收治的COPD合并Ⅱ型呼吸衰竭患者68例进行研究,随机数字表法分为观察组（n=34）和对照组（n=34）,对照组给予常规西医联合无创通气治疗,观察组在对照组基础上加用参附注射液治疗。比较两组临床疗效、肺功能指标（FEV₁、FVC、FEV₁/FVC）、心率、呼吸频率、血气指标（pH、PaCO₂、PaO₂）、无创通气时间及不良反应发生情况。结果:经过治疗后,观察组治疗总有效率为91.18%,高于对照组的70.59%,差异有统计学意义（P<0.05）;治疗前,两组FEV₁、FVC、FEV₁/FVC、心率、呼吸频率、pH、PaCO₂、PaO₂水平差异无统计学意义（P>0.05）,治疗后,两组FEV₁、FVC、FEV₁/FVC、pH、PaO₂水平明显升高,心率、PaCO₂及呼吸频率明显更优,观察组FEV₁、FVC、FEV₁/FVC、PaO₂水平高于对照组,观察组心率及呼吸频率慢于对照组,PaCO₂及无创通气时间优于对照组,差异有统计学意义（P<0.05）;两组均未见明显不良反应。结论:参附注射液治疗COPD合并Ⅱ型呼吸衰竭患者效果良好,可有效改善患者肺功能和血气指标,降低患者心率、呼吸频率,缩短无创通气时间。更多还原

【Abstract】 Objective:To investigate the efficacy and safety of Shenfu Injection in the treatment of chronic obstructive pulmonary disease （COPD） with type Ⅱ respiratory failure.Method:Sixty-eight patients with COPD and type Ⅱ respiratory failure treated in our hospital were selected for study.According to the random number table method they were divided into the observation group （n=34） and the control group （n=34）.The control group was given conventional western medicine combined with non-invasive ventilation,the observation group was treated with Shenfu Injection on the basic of the control group.Compared the clinical efficacy,lung function indicators （FEV₁,FVC,FEV₁/FVC）,heart rate,respiratory frequency,blood gas indicators （pH,PaCO₂,PaO₂）,non-invasive ventilation time and adverse effects between the two groups.Result:After treatment,the total effective rate in the observation group was 91.18%,which was higher than 70.59% in the control group,the difference was significant （P<0.05）.Before treatment,the levels of FEV₁,FVC,FEV₁/FVC,heart rate,respiratory rate and pH,PaCO₂,PaO₂ in the two groups were not significantly different （P>0.05）.After treatment,the levels of FEV₁,FVC,FEV₁/FVC,pH,PaO₂ significantly increased than those before treatment in both groups;heart rate,PaCO₂ and respiratory frequency were significantly better than those before treatment in both groups （P<0.05）.After treatment,the levels of FEV₁,FVC,FEV₁/FVC,PaO₂ in the observation group were higher than those in the control group,the heart rate and respiratory rate of the observation group were slower than those of the control group,PaCO₂ and non-invasive ventilation time of the observation group were better than those of the control group,the differences were statistically significant （P<0.05）.No significant adverse reactions were observed in both groups.Conclusion:Shenfu Injection has good curative effect on patients with COPD combined with type Ⅱ respiratory failure.It can effectively improve patients’ lung function and blood gas index,reduce heart rate and respiratory rate,shorten non-invasive ventilation time.更多还原
