血性胸腹水液基涂片中红细胞去除方法探讨

【摘要】 目的探讨去除疑似肿瘤血性胸腹水液基涂片、细胞块中红细胞的方法,为提高临床病理细胞学诊断阳性率提供参考。方法收集河南科技大学第三附属医院临床送检病理科的新鲜血性疑似肿瘤胸腹水标本65例为研究样本。每例分为对照组与实验组,实验组分别加入95%酒精、85%酒精、75%酒精、50%酒精、30%酒精、10%酒精,与不做任何处理并按照传统方法行液基涂片、细胞块的对照组进行比较。结果液基细胞学和组织沉渣包埋染色显示,随着酒精浓度的降低红细胞溶解呈上升趋势,当酒精浓度降低到50%以后,红细胞完全溶解,然后随着酒精浓度的进一步降低细胞开始出现退变,切片染色评分有统计学意义(P<0.01)。50%酒精处理组免疫组化阳性信号更强(P<0.05或0.01),肿瘤细胞阳性检出率显著升高(P<0.05或0.01)。结论 50%酒精能去除血性胸腹水液基涂片、细胞块背景中的红细胞,提高肿瘤血性胸腹水细胞学的阳性率。

【Abstract】 Objective To explore the method of removing red blood cells from liquid-based smear of bloody hydrothorax and ascites and cell blocks of suspected tumors, so as to provide reference for improving the positive rate of clinical pathological cytology diagnosis. Methods 65 cases of fresh bloody hydrothorax and ascites of suspected tumors from the department of pathology of The Third Affiliated Hospital of Henan University of Science and Technology were collected as study samples. Each case was divided into control group and experimental group. The experimental group were added with 95% alcohol, 85% alcohol, 75% alcohol, 50% alcohol, 30% alcohol, 10% alcohol, respectively, and compared with the control group which did not receive any treatment and performed liquid-based smear and cell block according to traditional method. Results Liquid-based cytology and tissue sediment embedding staining showed that erythrocyte dissolution increased with the decrease of alcohol concentration. When the alcohol concentration dropped to 50%, erythrocyte dissolved completely, and then cells began to degenerate with the further decrease of alcohol concentration, and the section staining score was statistically significant(P < 0.01). Immunohistochemical positive signal was stronger in the 50% alcohol treatment group(P < 0.05 or P < 0.01), and the positive detection rate of tumor cells significantly increased(P < 0.05 or P < 0.01). Conclusion 50% alcohol can remove red blood cells from the background of liquid-based smear of bloody hydrothorax and ascites and cell blocks, and improve the positive rate of cytology in hemothorax and ascites of tumor.
