鼻咽癌患者血浆EBV-miR-BART7-3p与 EBV-miR-BART17-5p的表达及其临床意义

【摘要】 目的检测鼻咽癌患者血浆中BART-miRNAs的表达情况,评价它们对鼻咽癌诊治的临床应用价值。方法采用反转录PCR检测111例首诊鼻咽癌患者、42例治疗后复发/转移鼻咽癌患者、22例非鼻咽癌患者及24名健康体检者血浆中BART-miRNAs,统计分析其与鼻咽癌患者临床特点的关系,以及治疗前后的变化,绘制ROC曲线,计算AUC,评估其作为鼻咽癌诊断工具的性能。结果鼻咽癌患者血浆中EBV-miR-BART7-3p与EBV-miR-BART17-5p相对表达水平显著高于非鼻咽癌患者与健康体检者(P<0.05),均与患者N分期和临床分期相关(P<0.05),且在治疗后显著下降(P<0.05),而治疗后转移患者相对表达水平显著升高。单独检测诊断鼻咽癌的AUC为0.829和0.833,联合检测诊断鼻咽癌的AUC为0.860。结论检测血浆中EBV-miR-BART7-3p与EBV-miR-BART17-5p,可为鼻咽癌诊疗以及分期等进一步研究提供有效依据。

【Abstract】 Objective To detect the expression of BART-miRNAs in plasma of patients with nasopharyngeal carcinoma, and evaluate their clinical application values in the diagnosis and treatment of nasopharyngeal carcinoma. Methods RT-PCR was used to detect BART-miRNAs in plasma of 111 patients with first diagnosed nasopharyngeal carcinoma, 42 patients with recurrent/metastatic nasopharyngeal carcinoma after treatment, 22 patients without nasopharyngeal carcinoma and 24 healthy controls. And then, relationship between BART-miRNAs and clinical characteristics of patients with nasopharyngeal carcinoma, as well as the changes before and after treatment, were statistically analyzed. ROC curve was drawn, AUC was calculated, and its performance as a diagnostic tool for nasopharyngeal carcinoma was evaluated. Results The relative expression levels of EBV-miR-BART7-3 p and EBV-miR-BART17-5 p in the plasma of nasopharyngeal carcinoma patients were significantly higher than those of patients without nasopharyngeal carcinoma and healthy people(P < 0.05), which were all correlated with N stage and clinical stage of patients(P < 0.05), and decreased significantly after treatment(P < 0.05), and the relative expression level of metastatic patients increased significantly after treatment. The AUC of single detection for diagnosis of nasopharyngeal carcinoma was 0.829 and 0.833, and that of combined diagnosis of was 0.860. Conclusion The detection of EBV-miR-BART7-3 p and EBV-miR-BART17-5 p in plasma can provide an effective basis for the diagnosis, treatment and staging of nasopharyngeal carcinoma.
