【摘要】 目的探讨以快速康复外科理念指导下,三孔腹腔镜直肠癌根治术与五孔腔镜直肠癌根治术的临床疗效差异。方法选取2017年6月—2018年4月在我院住院并行手术治疗患者45例,其中行"三孔"腹腔镜直肠癌根治术20例,常规五孔腹腔镜直肠癌根治术25例,对比分析两组临床疗效。结果两组在术中出血量、淋巴结清扫数目、术后首次排气时间、术后并发症、血清C反应蛋白(CRP)、白细胞(WBC)和白介素6(IL-6)、术后疼痛评分的差异没有统计学意义(P>0.05),"三孔"组手术时间较"五孔组"短(P <0.05)、手术耗材费用低(P <0.05)。术后并发症两组差异无统计学意义(P>0.05)。结论在快速康复外科理念指导下,三孔腹腔镜直肠癌根治术在术后并发症、术后应激反应、术后疼痛、术后胃肠功能恢复上较五孔腹腔镜直肠癌根治术没有明显优势,但三孔直肠癌根治术在降低手术耗材上具有一定优势。更多还原

【Abstract】 Objective To investigate the difference in clinical efficacy of five-port laparoscopic radical resection for rectal cancer and three-port laparoscopic radical resection for rectal cancer under the guidance of the concept of Fast Track Surgery. Methods A total of 45 patients admitted to our hospital for surgical treatment from June 2017 to April 2018 were selected.Among them,20 patients underwent three-port laparoscopic radical resection for rectal cancer,and 25 patients underwent conventional five-port laparoscopic radical resection for rectal cancer.The clinical efficacy of two groups was compared and analyzed. Results There were no statistically significant differences in intraoperative blood loss,number of dissected lymph nodes,first postoperative exsufflation time,postoperative complications,serum C-reactive protein(CRP),white blood cells(WBC)and interleukin-6(IL-6)and postoperative pain scores between two groups(P >0.05).The operation duration of the three-port group was shorter than that of the five-port group(P <0.05),and the cost of surgical consumables was lower than that of the five-port group(P <0.05).There was no significant difference in postoperative complications between two groups(P >0.05). Conclusion Under the guidance of the concept of Fast Track Surgery,three-port laparoscopic radical resection for rectal cancer has no significant advantages over five-port laparoscopic radical resection for rectal cancer in terms of postoperative complications,postoperative stress response,postoperative pain and postoperative gastrointestinal function recovery.However,three-port laparoscopic radical resection for rectal cancer has certain advantages in reducing the cost of surgical consumables.更多还原
