MRI不同序列对子宫内膜癌定性和定量诊断的价值

【摘要】 目的比较DWI与T2WI诊断子宫内膜癌敏感性以及测量病变大小准确性的差别,以更好地指导临床医师阅读MRI。方法 34例子宫内膜癌患者术前均已行多模态3.0T MRI检查,以术后病理为金标准,比较这两种序列诊断EC的灵敏度,同时分别比较两种序列测量癌灶上下径、左右径与术后径线的差别。结果 DWI诊断子宫内膜癌的敏感性(97%)高于T2WI(80%),DWI所测得癌灶上下径、左右径与术后均有差别(P上下径=0.002,P左右径=0.002),T2WI均无差别(P上下径=0.579,P左右径=0.261)。结论 DWI可明显提高MRI诊断子宫内膜癌的敏感性,而T2WI测量癌灶大小的准确性更高。

【Abstract】 Objective To compare the sensitivity of diagnosis and accuracy of size evaluation of DWI and T2WI in EC, so as to guide clinicians to read MRI better. Methods 34 patients with EC had been examined with 3.0 TMR before operation. The sensitivity of the two sequences in diagnosing EC was compared by using pathology as the golden standard. The longitudinal and transverse diameters measured on DWI and T2WI were compared with postoperative diameters respectively. Results The sensitivity of DWI in the diagnosis of EC(97%) was higher than that in T2WI(80%). The longitudinal and transverse diameters of T2WI were coincident with postoperative diameters. The longitudinal and transverse diameters of DWI were different from postoperation. Conclusion The sensitivity of MRI in the diagnosis of EC can be significantly improved by the use of DWI, and the accuracy of T2WI in measuring the tumor size is higher.
