MRA技术在颅内动脉瘤诊断进展

【摘要】 颅内动脉瘤(intracranial aneurysm, IA)系颅内动脉管壁的异常扩张或膨出,是非创伤性蛛网膜下腔出血的最常见原因。颅脑MRA具有非侵入性、无辐射、操作简单、可重复进行、费用低廉的特点;随着各种非对比增强MRA技术的发展,目前颅脑MRA在临床上已基本取代CT血管成像、DSA成为IA诊断与筛检的主要影像学检查手段。非对比4D-MRA技术除对IA进行形态学评价外,还可动态观察IA的血流动力学特征,有望对其疗效和预后进行更准确评估。本文对各种MRA技术在IA诊断的应用进展进行综述。

【Abstract】 The intracranial aneurysm(IA) is defined as an abnormal focal dilatation of intracranial artery. The rupture of IA represents the most common cause of nontraumatic subarchnoid hemorrhage(SAH). Intracranial magnetic resonance angiography(MRA) is noninvasive, non-radiative, and characterized by simple and repeatable operation with low costs. With the rapidly development of various non-contrast enhanced MRA techniques, MRA has by far become the most popular imaging method for the diagnosis and screening of IA, and has replaced CT angiography and DSA in clinical practice to a large extent. In recent years, non-contrast enhanced 4 D MRA has been applied to qualitatively and quantitatively assess the intracranial aneurysm hemodynamic alterations, and has become a promising method for evaluation of treatment efficacy and prognosis. This article reviews a variety of MRA techniques involved in the diagnosis of IA.
