Lemmel’s综合征的MSCT表现及诊断价值

【摘要】 目的探讨多排螺旋CT对Lemmel’s综合征的诊断价值。方法回顾分析35例经影像及临床证实的Lemmel’s综合征的MSCT平扫、增强及多种后处理重建成像表现。结果 35个十二指肠乳头旁憩室中,7个憩室囊腔内仅为气体,15个囊腔内为气液平,6个囊腔内为气液与食物混杂密度影,7个囊腔内仅为液体;15例患者行MSCT增强扫描憩室壁有不同程度强化;合并急性胆囊炎6例,胆囊及胆总管均有结石4例,单纯胆总管结石5例,单纯胆囊结石7例,胰腺炎13例;初诊有14例未能提出Lemmel’s综合征的诊断;后处理重建成像对十二指肠憩室及周围病变显示更清晰。结论MSCT是观察Lemmel’s综合征包括十二指肠乳头旁憩室、胆系、胰腺等病变最为简便而有效的技术;提高警惕利用MSCT后处理重建成像仔细观察有无乳头旁憩室的存在,是诊断本症的关键。

【Abstract】 Objective To investigate the diagnostic value of multi-slice spiral CT in Lemmel’s syndrome. Methods Retrospective analysis was conducted on 35 cases of imaging and clinically proven Lemmel’s syndrome with MSCT nonenhanced,enhanced and multipl post-processing techniques. Results Among the 35 duodenal papillary diverticulum, 7 were with only gas in the cystic cavity, 15 were with gas and fluid in the cystic cavity, 6 were gas, fluid and food in the cystic cavity, 7 were only fluids in the cystic cavity. The diverticulum wall of 15 patients underwent MSCT-enhanced scanning were with different degrees of enhancement; There were 6 cases of acute cholecystitis, 4 cases of gallbladder and common bile duct stones, 5 cases of simple bile duct stones, 7 cases of simple gallstones, 13 cases of pancreatitis; 14 cases of Lemmel’s syndrome were not diagnosed in the initial diagnosis; Post-processing reconstruction imaging showed clearer findings in the duodenal diverticulum and surrounding lesions. Conclusion MSCT is the most simple and effective technique for observing Lemmel’s syndrome including duodenal papillary diverticulum, biliary system, pancreas and other diseases. The key to diagnose Lemmel’s syndrome is observing the presence or absence of duodenal papillary diverticulum by MSCT post-processing reconstruction imaging.
