经颅直流电刺激对脑梗死偏瘫患者上肢功能及血清BDNF、NGF、NT-3水平的影响
Effects of transcranial direct current stimulation on upper limb function and serum levels of BDNF, NGF and NT-3 in patients with hemiplegia after cerebral infarction
【摘要】 目的研究经颅直流电刺激对脑梗死偏瘫患者上肢功能及血清BDNF、NGF、NT-3水平的影响。方法选取2018年2月至2019年2月四川绵阳四〇四医院收治的脑梗死偏瘫患者96例作为研究对象,按照随机数字表法将其分为对照组(48例)与试验组(48例)。对照组患者应用虚拟情景互动训练治疗,试验组患者在对照组的基础上联合经颅直流电刺激治疗。7 d为1个疗程,两组患者均连续治疗8个疗程。比较两组患者治疗前后神经功能、上肢功能及血清BDNF、NGF、NT-3水平。结果与治疗前比,治疗后两组患者NIHSS评分均降低,且试验组低于对照组;而两组患者ADL、Fugl-Meyer、ARAT评分,Brunnstrom上肢分期、手分期及血清BDNF、NGF、NT-3水平均升高,且试验组高于对照组(均P <0.05)。结论经颅直流电刺激可升高患者血清BDNF、NGF、NT-3水平,有助于修复缺损的脑神经,促进神经功能恢复,同时可有效改善脑梗死偏瘫患者的上肢功能,改善患者预后。
【Abstract】 Objective To study the effect of transcranial direct current stimulation on upper limb function and serum levels of BDNF, NGF and NT-3 in patients with hemiplegia after cerebral infarction. Methods From February 2018 to February 2019, 96 cases patients with with hemiplegia after cerebral infarction admitted to the Sichuan Mianyang 404 Hospital were selected as the research objects, the patients were randomly divided into the control group(48 cases) and the experimental group(48 cases) according to the random number table method. The control group was treated with virtual scene interactive training, the patients in the experimental group were treated combined with transcranial direct current stimulation on the basis of the control group. 7 d was a course of experimental, and the two groups were treated for 8 consecutive courses. The neurological function, upper limb function and serum levels of BDNF, NGF and NT-3 of patients in the in two groups were compared before and after experimental. Results Compared with before experimental, NIHSS scores of patients in the two groups decreased after experimental, and the experimental group was lower than the control group; ADL, Fugl Meyer score, ARAT score, Brunnstrom upper limb stage, Brunnstrom hand stage and serum BDNF, NGF and NT-3 levels of patients in the two groups increased, and the experimental group was higher than the control group(all P < 0.05). Conclusion Transcranial direct current stimulation can increase the serum levels of BDNF, NGF and NT-3, it is helpful to repair the brain nerve defect, and then improve the recovery of the nerve function, at the same time, it can effectively improve the upper limb function of patients with cerebral infarction hemiplegia, improve patients’ prognosis.
