【摘要】 目的了解我院氯吡格雷与质子泵抑制剂(proton pump inhibitor,PPI)联用的情况并分析其合理性,为我院规范化管理PPI提供依据,促进PPI的合理使用。方法随机抽取2019年1～6月同时使用氯吡格雷与PPI的病例300份,对患者的基本情况、联用PPI的种类进行统计分析,并点评联用的合理性。结果氯吡格雷与雷贝拉唑联用最多,占72.67%,其次为兰索拉唑、艾普拉唑、泮托拉唑、艾司奥美拉唑、奥美拉唑,而氯吡格雷与PPI联用不合理的病例有51例,其中无指征联用PPI预防的有36例,遴选PPI不适宜的8例,PPI用法用量不合理的有7例。结论我院氯吡格雷与PPI联用大部分符合指南及药品说明书推荐,但存在倾向性用药及不合理联用的情况,应加强PPI的管理与培训,增加PPI专项点评,尤其针对不合理使用PPI的临床科室。更多还原

【Abstract】 Objective To investigate the situation of combination of clopidogrel and proton pump inhibitor(PPI) in our hospital and analyze its rationality,so as to provide basis for the standardized management of PPI. Methods Through the medical record system,300 patients who used clopidogrel and PPIs simultaneously from January to June 2019 were enrolled,and the basic situation of patients,the type and rationality of PPI in combination with clopidogrel were analyzed.Results Clopidogrel was most commonly used in combination with rabeprazole,accounting for 72.67%,followed by lansoprazole,epprazole,pantoprazole,esomeprazole and omeprazole.There were 51 cases of unreasonable use of PPI in combination with clopidogrel,without indication in 36 cases,inappropriate in 8 cases,and unreasonable of usage and dosage in 7 cases.Conclusion The combined use of PPI with clopidogrel in our hospital mostly complies with the guidelines and drug instructions,but there also exists tendentious and irrational use of PPI.Management of PPI should be strengthened,and special comments on PPI should be added,especially for the clinical departments with unreasonable application of PPI.更多还原
