盆底康复治疗对良性妇科疾病行全子宫切除术患者盆底肌力、疲劳度及性功能的影响

【摘要】 目的探讨盆底康复治疗对良性妇科疾病行全子宫切除术患者盆底肌力、疲劳度及性功能的影响。方法选取2018年6月至2019年6月广东省深圳市龙华区人民医院妇产科收治的93例因良性妇科疾病需行全子宫切除术患者作为研究对象。随机分为观察组(n=47)和对照组(n=46)。对照组患者术后采取凯格尔训练,观察组患者术后采取凯格尔训练+电刺激+生物反馈治疗。比较两组盆腔器官脱垂情况、压力性尿失禁情况、盆底肌力、疲劳度、性生活质量。结果术后6个月,观察组患者盆腔脏器脱垂程度轻于对照组,差异具有统计学意义(P<0.05);观察组患者压力性尿失禁发生率明显低于对照组,差异具有统计学意义(P<0.05);观察组患者盆底肌力明显优于对照组,差异具有统计学意义(P<0.05);观察组患者Ⅰ类肌、Ⅱ类肌疲劳度均低于对照组,差异具有统计学意义(P<0.05);观察组患者性生活质量评分高于对照组,差异具有统计学意义(P<0.05)。结论凯格尔训练联合电刺激、生物反馈盆底康复治疗可显著提高全子宫切除术患者盆底肌力,改善盆底肌疲劳度,降低盆底功能障碍性疾病的发生风险,提高性生活质量。

【Abstract】 Objective To investigate the effect of pelvic floor rehabilitation on pelvic floor muscle strength, fatigue and sexual function in patients underwent total hysterectomy for benign gynecological diseases. Methods A total of 93 patients admitted to the Department of Obstetrics and Gynecology of Longhua District People′s Hospital in Shenzhen from June 2018 to June 2019 were randomly divided into observation group(n=47) and control group(n=46). Patients in the control group received postoperative Kegel training, while patients in the observation group received postoperative Kegel training + electrical stimulation + biofeedback therapy. Pelvic organ prolapse, stress urinary incontinence, pelvic floor muscle strength, fatigue, and sexual quality were compared between the two groups. Results Six months after surgery, pelvic organ prolapse in the observation group was less severe than that in the control group, and the difference was statistically significant(P<0.05). The incidence of stress incontinence in the observation group was significantly lower than that in the control group, with statistically significant difference(P<0.05). The pelvic floor muscle strength of the observation group was significantly better than that of the control group, and the difference was statistically significant(P<0.05). Type Ⅰ muscle fatigue and type Ⅱ muscle fatigue in observation group were lower than that in control group(P<0.05). The score of sexual quality in the observation group was higher than that in the control group, and the difference was statistically significant(P<0.05). Conclusions Kegel training combined with electrical stimulation, biofeedback and other pelvic floor rehabilitation therapy can significantly improve pelvic floor muscle strength, ameliorate pelvic floor muscle fatigue, reduce the risk of pelvic floor dysfunction disease, and improve the quality of sexual life in patients with total hysterectomy.
