低频电刺激激活NGF/TRKA信号改善膀胱过度活动综合征患者功能障碍的临床研究

【摘要】 目的研究低频电刺激激活NGF/TRKA信号改善膀胱过度活动综合征患者功能障碍的临床机制。方法筛选从2017年1月至2019年1月在东莞市人民医院妇产科接受治疗的80例膀胱过度活动综合征功能障碍患者作为研究对象。随机分为对照组(常规康复治疗,n=40)和观察组(n=40)。除观察组采用低频电刺激辅助治疗外,两组实验对象医疗环境和康复手段一致,避免干扰因素造成的数据异常或者不准确。通过RNA提取和实时PCR分析检测NGF和TRKA RNA表达;通过蛋白质印迹分析血清中NGF和TRKA蛋白表达情况;通过低频电刺激对观察组患者进行辅助电疗;通过膀胱功能检测评定患者膀胱功能恢复情况。结果与对照组膀胱内压(19.36±3.52)mmH2O,排尿次数(3.37±0.01)天/次,单次排尿量(168.37±6.62)mL相比,观察组(31.25±2.16)mmH2O,(6.72±0.03)天/次,(234.31±5.56)mL升高,差异有统计学意义(P<0.05);在NGF和TRKA蛋白表达上,观察组(3.28±0.31,3.16±0.15)较对照组(1.21±0.24)(1.05.±0.17)升高,比较有统计学意义(P<0.05);在NGF和TRKAMRNA表达上,观察组(2.67±0.21,2.43±0.11)较对照组(0.98±0.05,1.01±0.03)升高,差异有统计学意义(P<0.05)。结论低频电刺激可以激活NGF/TRKA信号通路,改善膀胱过度活动综合征患者功能障碍。

【Abstract】 Objective To study the clinical mechanism of low-frequency electrical stimulation activating NGF/TRKA signal to improve the dysfunction of patients with overactive bladder syndrome. Methods From January 2017 to January 2019, 80 patients with bladder hyperactivity syndrome were selected and randomly divided into control group(routine rehabilitation therapy, n=40) and observation group(low-frequency electrical stimulation adjuvant therapy based on routine rehabilitation treatment, 15 minutes each time, 3 times a day, continuous treatment for a month, n=40). In addition to the observation group using low-frequency electrical stimulation as an adjuvant therapy, the medical environment and rehabilitation methods of the two groups were consistent, so as to avoid abnormal or inaccurate data caused by interference factors. The expression of NGF and TRKARNA was detected by RNA extraction and real-time PCR analysis. The expression of NGF and TRKA in serum was analyzed by Western blotting; the patients in the observation group were treated by low-frequency electrical stimulation; and the recovery of bladder function was assessed by bladder function test. Results Compared with the control group, the intravesical pressure [(19.36±3.52) mmH2O vs.(31.25±2.16) mmH2O], the number of times of micturition [(3.37±0.01) days/time vs.(6.72±0.03) days/time], the volume of single micturition [(168.37±6.62) mL vs.(234.31±5.56) mL] increased significantly in the observation group, all with statistically significant differences(P<0.05). The expression of NGF and TRKA protein in the observation group [(3.28±0.31),(3.16±0.15)] was higher than that in the control group [(1.21±0.15),(1.05±0.17)], all with statistically significant differences(P<0.05). The expression of NGF and TRKAMRNA in the observation group [(2.67±0.21),(2.43±0.11)] was higher than that in the control group [(0.98±0.05),(1.01±0.03]), all with statistically significant differences(P<0.05). Conclusions Low-frequency electrical stimulation can activate NGF/TRKA signaling pathway and improve the dysfunction of patients with overactive bladder syndrome.
