肝移植术后下腔静脉和肝静脉并发症的血管内介入治疗

【摘要】 目的探讨肝移植术后下腔静脉和肝静脉并发症的血管内介入治疗。方法选取2012年12月至2019年3月昆明市第一人民医收治肝移植术后因下腔静脉和肝静脉并发症接受介入治疗的7例患者。观察术后患者病变血管通畅情况、肝功能、下肢水肿及腹腔积液情况。结果 6例行球囊扩张及支架置入治疗,1例肝移植术后第二天出现腔静脉肝后段明显狭窄,给予支架置入治疗,未行球囊扩张;5例术后1~2周下肢水肿明显缓解,腹腔积液明显减少,均取得了较好的效果;2例死亡。结论血管内介入治疗是肝移植术后下腔静脉和肝静脉并发症的一种较安全、有效的方法。

【Abstract】 Objective To explore the interventional treatment of the complications of inferior vena cava and hepatic vein after liver transplantation. Method From December 2012 to March 2019, the first people’s hospital of Kunming was selected to treat 7 patients who received interventional treatment due to complications of vena cava and hepatic vein after liver transplantation. The patency of pathological vessels, liver function, lower extremity edema and ascites were observed. Result Six patients were treated with balloon dilatation and stent implantation. One patient had obvious stenosis of the posterior segment of the vena cava on the second day after liver transplantation, and was treated with stent implantation without balloon dilatation. Five patients had significant relief of lower extremity edema and ascites at 1-2 weeks after operation, and achieved good results. Two patients died. Conclusion Percutaneous intravascular intervention is a safe and effective method for the treatment of inferior vena cava and hepatic vein complications after liver transplantation.
