【摘要】 目的总结肠造瘘患者在确诊至术后6个月纵向的情绪体验,为针对性制订有效的心理护理干预措施提供依据。方法运用质性研究中的现象学研究法对桂林市某三甲医院肠造瘘患者确诊后至手术前、术后至出院前、出院后3个月及出院后3至6个月进行半结构访谈,根据Colaizzi 7步分析法对资料进行分析整理,并提炼主题。结果肠造瘘患者确诊后至术前情绪主题:疾病不确定感、内疚和抑郁和饮食、睡眠受限的焦虑。肠造瘘患者术后至出院前情绪主题:疾病不确定感、内疚和抑郁、睡眠缺乏的焦虑和自我照护技能缺乏的担忧。肠造瘘患者出院后3个月内情绪主题:自尊心受挫、自我照护技能缺乏的担忧和心理内疚和抑郁。肠造瘘患者出院后3～6个月情绪主题:创伤后成长和对未来的担忧。结论肠造瘘确诊患者在不同时期的情绪状态呈现不同特征。医务人员应关注不同阶段肠造瘘患者的不良情绪反应,针对性采取措施促进患者情绪的正向转变,克服肠造口带来的身体和生活限制,改善其生活质量。更多还原

【Abstract】 Objective To investigate the emotional conditions of patients with enterostomy at different stages and explore the influencing factors so as to provide basis for effective psychological nursing interventions.Methods Phenomenological research method was used in the qualitative research.The semi-structured interviews were conducted among the patients with enterostomy in a general hospital of Guilin right after diagnosis,before operation,after operation,before discharge,3 months and 3-6 months after discharge.The acquired data were analyzed and sorted out using Colaizzi 7-step analysis method.Results Before operation,the emotional states of patients with enterostomy included diseas uncertainty,psychologicaguilt,depression from restrained diet and less sleep.From post-operation to pre-discharge,the states were mainly disease uncertainty,guilt,depression,anxiety from less sleep and lack of self-care skills.Within 3 months after discharge,the states mainly were lack of self-esteem,lack of self-care skills,guilt and depression.Three to six months after discharge,the states were post-tramatic grouth and wony about the future.Conclusions The patients with enterostomy may ran into different emotional states at different periods.Therefore,medical staff should pay attention to the actual emotions at different stages,taking target measures to help them cope with the problems from enterostomy and improve their quality of life.更多还原
