腹腔镜辅助下改良Soave术治疗小儿先天性巨结肠的临床研究
Clinical study of laparoscopic assisted transanal gradient resection of rectum muscle sheath modified Soave procedure for the treatment of Hirschsprung’s disease in children
【摘要】 目的:探讨腹腔镜辅助下改良Soave术治疗小儿先天性巨结肠的临床疗效。方法:2017年4月-2020年5月收治先天性巨结肠患儿74例,随机分为两组,各37例。对照组给予常规经腹腔镜辅助下Soave术治疗;试验组给予腹腔镜辅助下改良Soave术治疗。比较两组疗效。结果:试验组手术及术后各指标恢复情况均显著优于对照组,差异有统计学意义(P<0.05)。试验组术后血清IL-8、hs-CRP、TNF-α水平均显著低于对照组,Krickenbeck、Heikkinen评分均显著高于对照组,差异均有统计学意义(P<0.05)。结论:采用腹腔镜辅助下改良Soave术治疗小儿先天性巨结肠可减轻对患儿机体造成的手术创伤及炎症反应,提高机体肛门功能及排便功能,促进患儿身体康复。
【Abstract】 Objective:To investigate the clinical effect of laparoscopic assisted transanal gradient resection of rectum muscle sheath modified Soave operation in the treatment of Hirschsprung’s disease in children.Methods:From April 2017 to May 2020,74 children with Hirschsprung’s disease were selected,they were randomly divided into the two groups with 37 cases in each group.The control group was treated with conventional laparoscopic assisted Soave operation.The study group was treated with laparoscopic assisted modified Soave operation.We compared the efficacy of the two groups.Results:The operation and postoperative recovery indexes of the study group were significantly better than those of the control group,the differences were statistically significant(P<0.05).The postoperative serum IL-8,hs-CRP,and TNF-α levels in the test group were significantly lower than those in the study group,and the Krickenbeck score and Heikkinen score were significantly higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Laparoscopic-assisted transanal gradient resection of rectal muscle sheath modified Soave procedure for Hirschsprung’s disease in children can reduce the surgical trauma,reduce the inflammatory reaction,improve the anal function and defecation function,and promote the physical recovery of children.
