【摘要】 目的:探讨糖尿病肾病患者中医证型与血清蛋白电泳各蛋白成分的关系。方法:2017年5月-2018年5月收治糖尿病肾病患者150例,按气阴两虚证、肝肾阴虚证、气血两虚证、脾肾阳虚证四型分组,检测血清前白蛋白和蛋白电泳各蛋白成分,分析中医证型与血清蛋白的关系。同时选取35例正常体检者作为正常对照组。结果:150例糖尿病肾病患者中气阴两虚证84例,肝肾阴虚证26例,气血两虚证10例,脾肾阳虚证30例,各中医证型中所有白球比值、白蛋白和前白蛋白中除气血两虚证外,比正常对照组降低,差异有统计学意义(P<0.05);α1球蛋白气阴两虚证和脾肾阳虚证两者之间有差异,同时此两证型比正常对照组均增高,差异有统计学意义(P<0.05);α2球蛋白脾肾阳虚证比正常对照组增高,差异有统计学意义(P<0.05);γ球蛋白在肝肾阴虚证和脾肾阳虚证比正常对照均增高,差异有统计学意义(P<0.05)。结论:糖尿病肾病的血清蛋白代谢异常主要表现在前白蛋白和白蛋白的降低、α1、α2、γ等球蛋白的百分比增高,提示在各中医证型中表现为营养状况差,血管炎性因子的存在和脂质成分代谢的异常,各中医证型的血清蛋白电泳为临床进行辨证施治提供一定的客观依据。更多还原

【Abstract】 Objective:To explore the relationship between TCM syndrome types and protein components of serum protein electrophoresis in patients with diabetic nephropathy.Methods:150 patients with diabetic nephropathy were selected from May2017 to May 2018,they were divided into four groups according to different types:deficiency of both qi and yin,liver-kidney yin deficiency,qi and blood deficiency,yang deficiency of spleen and kidney.The serum prealbumin and protein electrophoresis of various protein components were detected.Results:There were 84 cases of deficiency of both qi and yin,26 cases of liver-kidney yin deficiency,10 cases of qi and blood deficiency,30 cases of yang deficiency of spleen and kidney.The ratio of albumin to globulin,albumin and prealbumin in all TCM syndromes were lower than those in the normal control except for deficiency of qi and blood,the difference was statistically significant(P<0.05).There are differences between deficiency of both qi and yin and yang deficiency of spleen and kidney in α1 globulin,and the two syndrome types are higher than those in normal control group,the differences were statistically significant(P<0.05).The α2-globulin in yang deficiency of spleen and kidney was higher than that of normal control,the difference was statistically significant(P<0.05).The level of γ-globulin in liver-kidney yin deficiency and yang deficiency of spleen and kidney was higher than that in normal control,the difference was statistically significant(P<0.05).Conclusion:The abnormal metabolism of serum protein in diabetic nephropathy is mainly manifested in the decrease of prealbumin and albumin,the percentage of α1,α2,γ and other globulins increased.It is suggested that in the TCM syndromes,the nutritional status is poor,the existence of vascular inflammatory factors and the abnormal metabolism of lipid components.The serum protein electrophoresis of various TCM syndromes provides a certain objective basis for clinical dialectical treatment.更多还原
