【摘要】 目的探讨枸橼酸钠抗凝在高危出血患者血液净化治疗中的使用价值。方法选取2018年8月—2019年6月间我院收治的需行连续血液净化(continuous blood purification,CBP)的高危出血患者共62例,将其以随机数字表法的分组形式均分为试验组及对照组,每组31例。给予试验组患者局部枸橼酸钠抗凝技术联合CBP进行治疗,给予对照组患者体外局部体外低分子肝素抗凝联合CBP治疗,对比观察两组患者治疗前后血肌酐(serum creatinine,Scr)、尿素氮(blood urea nitrogen,BUN)水平,滤器凝血总发生率、静脉壶凝血总发生率及并发症出现情况等指标。结果试验组患者治疗后Scr及BUN明显优于对照组患者,差异具有统计学意义(P <0.05);试验组患者滤器凝血及静脉壶凝血总发生率低于对照组,差异显著(P <0.05);试验组并发症出现情况少于对照组患者,差异显著(P <0.05)。结论使用局部枸橼酸钠抗凝联合CBP可有效提高高危出血患者血液净化效果,减少并发症产生,值得于临床中进一步推广使用。更多还原

【Abstract】 Objective To explore the value of sodium citrate anticoagulation in blood purification treatment of high-risk bleeding patients. Methods Selected 62 patients with high-risk bleeding requiring continuous blood purification(continuous blood purification, CBP) between August 2018 and June 2019, divided into experimental group and control group, 31 cases in each group. The experimental group was treated with local sodium citrate anticoagulation technology combined with CBP, the control group were treated with local low molecular heparin anticoagulation and CBP in vitro. Serum creatinine(Scr), blood urea nitrogen(BUN), were compared before and after treatment. Results Scr and BUN in the experimental group were significantly good, and the difference was statistically significant(P < 0.05). The total incidence of filter coagulation and intravenous coagulation in the experimental group was lower, the differences were significant(P < 0.05); the complications had a significant difference(P < 0.05). Conclusion The use of local sodium citrate anticoagulation combined with CBP can effectively improve the blood purification effect and reduce the occurrence of complications in patients with high-risk bleeding. It is worthy of further clinical application.更多还原
