【摘要】 目的:探讨微创治疗老年骨质疏松性NeerⅡ型肱骨近端骨折的临床效果。方法:选取本院2016年1月～2018年12月共40例NeerⅡ型肱骨近端骨折合并骨质疏松老年患者进行回顾性分析,分别采取非手术治疗或经皮微创内固定技术(MIPO)治疗方案,并观察两组患者治疗前(入院时)和末次随访时Constant-Murley肩关节评分、视觉模拟评分(VAS)和健康指数评分(EQ-5D),以及骨折愈合时间等情况。结果:两组患者末次随访时的Constant-Murley评分、VAS评分、EQ-5D评分均优于治疗前,差异有统计学意义(P<0.05);且微创组末次随访时Constant-Murley评分优于非手术组(P<0.05),但两组患者在骨折愈合时间、VAS评分、EQ-5D评分方面无统计学差异(P>0.05)。结论:针对老年骨质疏松性NeerⅡ型肱骨近端骨折,MIPO手术方式可以有效改善患者伤后早期关节功能及生活质量。更多还原

【Abstract】 Objective:To evaluate the clinical efficacy of minimally invasive treatment of Neer type Ⅱ proximal humeral fractures from osteoporosis in elderly patients.Methods:Retrospective analysis was performed in 40 elderly cases of Neer type Ⅱ proximal humeral fractures concomitant with osteoporosis treated in our hospital between January 2016 and December 2018.The patients underwent either expectant treatment or minimally invasive percutaneous osteosynthesis(MIPO) technique using locking compression plate.The clinical indexes,including the shoulder joint scoring on Constant-Murley scale on admission and final follow-up,visual analogue scale(VAS) and health-related quality of life scores(EQ-5 D) as well as fracture healing time,were maintained and analyzed.Results:The scoring on Constant-Murley scale,VAS and EQ-5 D was greatly improved in the two groups by the final follow-up(P<0.05),and the scoring on Constant-Murley scale was better in patients treated with MIPO than those by expectant treatment(P<0.05).However,the difference remained insignificant in both groups regarding duration of fracture healing,and scoring on VAS and EQ-5 D(P>0.05).Conclusion:MIPO technique using internal plate fixation can be effective for Neer Ⅱ proximal humeral fractures from osteoporosis in the elderly,and better improve their joint function and quality of life following surgery.更多还原
