超重及肥胖对胚胎玻璃化冷冻损伤的影响

【摘要】 目的研究超重/肥胖对胚胎玻璃化冷冻损伤影响。方法纳入2013年5月至2015年12月于我中心行IVF/ICSI-ET患者,共5466周期,其中新鲜移植3 254周期,解冻移植2 212周期,根据体质量指数(body mass index,BMI)分为低体重组(<18.5 kg/m~2)、正常体重组(18.5~23.9 kg/m~2)、超重组(24.0~27.9 kg/m~2)及肥胖组(≥28.0 kg/m~2),采用Logistics回归模型去除混杂因素后分析不同BMI分组在新鲜移植和解冻移植对活产率、临床妊娠率及流产率的影响。结果在新鲜移植周期中,相对正常体重组,超重明显降低活产率(OR=0.77,95%CI:0.63~0.94)及临床妊娠率(OR=0.80,95%CI:0.65~0.98),对流产率无显著影响(OR=0.91,95%CI:0.62~1.33);玻璃化冷冻解冻移植周期,未发现超重对活产率(OR=0.94,95%CI:0.73~1.21)、临床妊娠率(OR=1.02,95%CI:0.80~1.30)及流产率有明星影响(OR=0.73,95%CI:0.43~1.24);在新鲜及解冻移植周期中,均未发现肥胖对妊娠结局的不良影响。结论超重/肥胖并不加重胚胎玻璃化冷冻损伤。

【Abstract】 Objective To investigate the effects of overweight/obesity on vitrification injury.Methods5 466 cycles of IVF/ICSI-ET,including 3 254 cycles of fresh embryo transfer and 2 212 cycles of thawing embryo transfer were collected and admitted to this study in our center from May 2013 to December 2015.According to body mass index(BMI),they were divided into low weight group(<18.5 kg/m~2),normal weight group(18.5~2.3.9 kg/m~2),overweight group(24.0~27.9 kg/m~2) and obesity group(> 28.0 kg/m~2).After adjusting the confounding factors,logistic regression model was used to analyze the effects of BMI on live birth rate,clinical pregnancy rate and abortion rate after fresh and thawed embryo transfer.Results Compared with normal weight group,overweight significantly reduced the live birth rate(OR=0.77,95% CI:0.63~0.94) and clinical pregnancy rate(OR=0.80,95% CI:0.65~0.98),but had no significant effect on the abortion rate(OR=0.91,95% CI:0.62~1.33).During the thawed embryo transfer cycle,there were no significant effect of overweight on the live birth rate(OR=0.94,95% CI:0.73~1.21),clinical pregnancy rate(OR=1.02,95% CI:0.65~0.98) and abortion rate(OR=0.73,95% CI:0.43~1.24).Obesity was found to have no adverse effects on pregnancy outcomes in both fresh and thawed embryo transfer cycles.Conclusion Overweight/obesity does not aggravate vitrification injury.
