磁共振成像弥散加权成像中表观弥散系数值对肝癌诊断的应用研究

【摘要】 目的探讨磁共振成像(MRI)中的弥散加权成像(DWI)技术在肝癌诊断中的应用价值。方法以病理结果为金标准,收集于我院2019年2月1日至3月4日行肝胆脾磁共振检查的肝癌患者34例。对其磁共振影像进行处理,对同一患者病变肝区和正常肝区的T1信号、T2信号、动态增强及DWI信号进行对比,分别记录正常肝区和病变肝区图像表现及表观弥散系数(ADC)均值进行t检验分析;并对病变区ADC值和患者的血清甲胎蛋白(AFP)值进行Pearson相关分析。结果经过对比,肝癌病变区ADC值为(8.0±5.3)×10-4mm2/s,正常肝区ADC值为(13.7±2.1)×10-4mm2/s,病变区ADC值比正常值明显降低(P<0.05);血清AFP值与ADC值相关系数(r=-0.172),两者无线性相关性(P>0.05)。结论核磁弥散加权成像技术对肝癌病变具有较高的诊断特异性,ADC值与AFP含量无相关性,血液AFP含量不能作为肝癌诊断的量化标准。

【Abstract】 Objective To explore the application value of dispersion weighted imaging(DWI)in the diagnosis of hepatocellular carcinoma. Methods Based on the pathological results,34 patients with hepatocellular carcinoma who underwent liver magnetic resonance examination in our hospital from February 1 to March 4,2019 were collected. The magnetic resonance imaging(MRI)was processed and the T1,T2,dynamic contrast enhancement and DWI signals in the lesion and normal liver regions of the same patient were compared. The mean and variance of apparent diffusion coefficients(ADC)in normal and pathological liver regions were recorded and compared in t test. We performed Pearson correlation analysis between ADC values in lesion area and serum alpha fetoprotein(AFP)values in patients. Result After comparison,the apparent dispersion coefficient in the lesion area of liver cancer was(8.0±5.3)×10-4 mm2/s. The apparent dispersion coefficient in the normal liver area was(13.7±2.1)×10-4 mm2/s. The ADC value in lesion area was significantly lower than that in normal area(P <0.05). The correlation coefficient between AFP content and ADC value showed no significant correlation(r =-0.172,P >0.05). Conclusion DWI has a high diagnostic specificity for hepato-cellular carcinoma lesions. There is no correlation between ADC value and AFP content. Blood AFP content can not be used as a quantitative criterion for the diagnosis of hepatocellular carcinoma.
