基于磁化转移技术的三维时间飞跃磁共振血管成像在缺血性脑血管疾病诊断中应用价值
The application value of 3D-MT-TOF MRA technology in the diagnosis of ischemic cerebrovascular diseases
【摘要】 目的探讨基于磁化转移技术的三维时间飞跃法磁共振血管成像技术(3D-MT-TOF-MRA)在颅内血管狭窄诊断中的价值。方法收集2017年6月至2019年8月符合缺血性脑血管疾病患者40例,所有患者均行基于磁化转移技术的3D-MT-TOF-MRA、TOF-MRA及数字减影血管造影(DSA)检查。以DSA、3D-MT-TOFMRA对照,对比分析3D-MT-TOF-MRA判断颅内血管狭窄的敏感度、特异度、准确度。结果所有患者均顺利完成检查,观察期内无不良事件发生。40例患者基于磁化转移技术的3D-MT-TOF-MRA检出中小血管狭窄27例(27/40,68%),DSA检出30例(30/40,75%),3D-MT-TOF-MRA检出血管狭窄20例(20/40,50%),3种方法诊断颅内中小血管狭窄差异有统计学意义(P<0.05)。以DSA为对照,基于磁化转移技术的3D-MT-TOF-MRA检出颅内中小血管狭窄的敏感度为87%,特异度为90%;2种方法诊断颅内中小血管狭窄差异无统计学意义(P>0.05),进一步κ系统检验,两种方法吻合度一般(κ=0.637)。结论 3D-MT-TOF-MRA技术与DSA在判断颅内动脉狭窄上具有较高一致性,且具有经济、安全等优点,值得临床进一步研究。
【Abstract】 Objective To investigate the application value of 3 D-MT-TOF-MRA technology in the diagnosis of ischemic cerebrovascular diseases. Methods Forty cases of ischemic cerebrovascular diseaseswere invloved in this study, in which all patients were performed routine abdominal TOF-MRA, DSA and 3 D-MT-TOF-MRA. The sensitivity, specificity and accuracy of 3 D-MT-TOF MRA technology in the diagnosis of ischemic cerebrovascular diseaseswere compared and analyzed, on the control of data from 3 D-TOF-MRA, DSA. Results All patients successfully completed the examination, and no adverse events occurred during the observation period.In 40 cases of ischemic cerebrovascular diseases, the 3 D-MT-TOF-MRA detected 27 cases vascular stenosis(27/40,68%), compared to 30 cases in DSA(30/40,75%), 20 cases in 3 D-MT-TOF-MRA(20/40,50%). There were evident statistic significance among three image technology in vascular stenosisdetected(P<0.01). Compared with DSA, the sensitivity of3 D-MT-TOF-MRA was 87%, specificity was 90% and Kappa value was 0.637. Conclusion 3 D-MT-TOF MRA technologyhas high consistency with DSA in the diagnosis of ischemic cerebrovascular diseases, with merits of safety and economy,which deserves further clinical research.
