【摘要】 目的探析存在妊高症的产妇剖宫产术后镇痛时,超声引导下腹横筋膜阻滞方案与局部浸润麻醉方案的镇痛效果。方法选择连云港市妇幼保健院妇产科收入的剖宫产产妇74例入组,均存在妊高症疾病,随机划分为两组,一组为对照组,采用切口局部浸润麻醉;一组为观察组,应用超声引导下腹横筋膜阻滞;比较两组术后镇痛效果,记录两组术后24 h舒芬太尼用量、术后第1次按压镇痛泵时间。结果应用视觉模拟评分评估疼痛程度可知,观察组在术后6 h、12 h术后的VAS评分明显低于对照组(P<0.05);观察组的24 h芬太尼用量少于对照组(P<0.05);观察组术后镇痛泵使用时间长于对照组(P<0.05)。结论相比于切口局部浸润麻醉而言,存在妊高症的产妇剖宫产术后镇痛应用超声引导下腹横筋膜阻滞效果效果更理想,具有良好的镇痛效果,作用时间更长。更多还原

【Abstract】 Objective To explore the analgesic effect of ultrasound-guided transversal abdominal fascia block and local infiltration anesthesia in patients with pregnancy induced hypertension after cesarean section. Methods 74 cases of cesarean section parturients from obstetrics and Gynecology of the hospital were selected and randomly divided into two groups, one group was the control group, using incision local infiltration anesthesia; the other group was the observation group, using ultrasound-guided transverse abdominal fascia block; the postoperative analgesic effect of the two groups was compared, and the dosage of sufentanil and the time of pressing analgesia pump for the first time after operation were recorded. Results The VAS score of the observation group was significantly lower than that of the control group at 6 h and 12 h after operation(P<0.05); the dosage of fentanyl in the observation group was less than that in the control group(P<0.05); the use time of analgesia pump in the observation group was longer than that in the control group(P<0.05). Conclusion Compared with incision local infiltration anesthesia, the effect of ultrasound-guided transversal abdominal fascia block on postoperative analgesia of pregnant women with pregnancy induced hypertension is better, with good analgesic effect and longer action time.更多还原
