有氧运动联合抗阻运动对腹膜透析患者微炎症及T细胞亚群的影响

【摘要】 目的探究有氧运动联合抗阻运动对持续不卧床腹膜透析患者微炎症状态、免疫功能及运动耐力的影响。方法 2018年1月至2019年12月沈阳市红十字会医院及中国医科大学附属盛京医院基线指标无显著性差异的腹膜透析患者72例随机分为对照组(n=36)、运动组(n=36)。运动组在对照组常规治疗的基础上,给予有氧运动联合抗阻运动训练,每周3次,每次30~60 min,共24周。对比运动训练前后两组患者微炎症状态、T细胞亚群、免疫球蛋白、运动耐力指标的变化。结果运动组运动训练24周后,微炎症指标C-反应蛋白、白细胞介素6较入组时及24周后对照组降低(P<0.05);免疫功能T细胞亚群CD4~+、CD8~+、CD4~+/CD8~+、免疫球蛋白IgG,运动耐力6 min步行试验、1 min坐立试验及握力均有所改善(P<0.05)。结论有氧运动联合抗阻运动可以改善腹膜透析患者微炎症状态及免疫功能;提高机体活动耐力;个体化的运动处方耐受性良好。

【Abstract】 ObjectiveTo explore the influence of aerobic exercise combined with resistance exercise on microinflammation, immune function and physical endurance in patients with continuous ambulatory peritoneal dialysis.Methods From January 2018 to December 2019, 72 peritoneal dialysis patients in Shenyang Red Cross Hospital and Shengjing Hospital of China Medical University were randomly divided into control group(n=36) and exercise group(n=36). The exercise group was given aerobic exercise combined with resistance training in addition to the conventional treatment for control group, 3 times a week, 30-60 minutes each time, a total of 24 weeks. After exercise training for 24 weeks, the changes of indicators of micro-inflammation, immune function, and 6 minutes walking test, 1 minute sitting test and grip strength were observed.ResultsAfter 24 weeks of aerobic exercise combined with resistance exercise training, the microinflammatory index(c-reactive protein, interleukin-6) in the exercise group, was lower than that of baseline and the control group(P<0.05). CD4+, CD8+, CD4+/CD8+,Ig G,6-minute walking test,1-minute sitting test and grip strength were improved(P<0.05).ConclusionAerobic exercise combined with resistance exercise can improve the state of micro-inflammation and immune function, and increase physical activity endurance in PD patients. Individualized exercise prescription is safe and well-tolerated.
