超声诊断胆囊息肉的临床价值

【摘要】 胆囊息肉的发病率占全球成人的0.3%~12.3%。传统经腹超声是诊断胆囊息肉的首选影像学检查方法,但其主要局限性在于对息肉血管的评估能力较低,从而无法判断胆囊息肉的性质。虽然多普勒超声在一定程度上克服了这些不足,但仍有局限性。超声对比剂的使用和对比增强成像技术的发展提高了传统超声的性能,可以准确评估胆囊息肉内的微循环,从而提高了鉴别胆囊息肉性质的能力。内镜超声能够使探头更接近目标组织,可以提高诊断的准确性,但由于是侵入性检查,所以比其他所有形式的经腹超声都有更高的并发症风险。由于易引起胆汁性腹膜炎等,经内镜或经腹超声引导下细针穿刺活检并非是鉴别胆囊息肉的常用操作方法。一些基于超声的评分系统是鉴别胆囊息肉的有效手段。

【Abstract】 The incidence of gallbladder polyps(GPs) occurred for 0.3%~12.3% of the global adult population. Mode Conventional ultrasound(CUS) is the first imaging investigation performed for the diagnosis of GPs, but the main limitation of CUS is a low ability to assess the microcirculation of GPs and therefore, it is impossible to judge the nature of GPs. Although Doppler ultrasound overcomes these shortcomings to a certain extent, it still has limitations. The introduction of ultrasound contrast agents and the development of contrast-specific imaging techniques have improved the performance of CUS, they give an accurate assessment of the blood flow within the lesion thereby to improve the ability to identify the nature of GPs. Endoscopic ultrasonography(EUS) enables the transducer to be in closer proximity to the target tissue, which can improve the accuracy of diagnosis, and however, it is an invasive examination that presents a higher risk of complications than all other forms of CUS. Because it is easy to cause biliary peritonitis, fine needle aspiration biopsy guided by endoscopy or transabdominal ultrasound is not a common operation method to identify GPs. Some ultrasound-based scoring systems could be useful means for differentiating between neoplastic GPs and pseudopolyps.
