【摘要】 目的 比较三维伪连续动脉自旋标记(3D pcASL)和CTP在评估颈动脉内膜切除术(CEA)前后脑血流灌注改变的一致性。方法 29例诊断为单侧颈动脉重度狭窄并拟行CEA的患者分别于术前及术后1周内行3D pcASL及CTP扫描,获得脑血流量(CBF)图,选择大脑中动脉供血区测量CBF值。分别比较术前、术后基于CTP和3D pcASL的CBF值变化情况,并评估基于2种方法获得的脑血流量变化率(ΔCBF)相关性。结果 (1)CTP:与术前比较,术后术侧和对侧CBF值均升高。(2)3D pcASL:与术前比较,术后术侧和对侧CBF值均升高。(3)相关分析提示基于2种方法测量的ΔCBF呈正相关。结论 3D pcASL对于CEA前后脑血流灌注的评估效果与CTP相似,可用于定量评估CEA前后的血流动力学改变。更多还原

【Abstract】 Objective To compare the consistency between three dimensional pseudocontinuous arterial spin labeling(3D pcASL)and CTP in evaluating changes of cerebral blood before and after carotid endarterectomy(CEA).Methods Twenty-nine patients diagnosed as unilateral severe carotid stenosis and scheduled for CEA were recruited.3D pcASL and CTP were performed one week before and after CEA respectively to obtain the cerebral blood flow(CBF) maps.CBF values were measured in the supply area of middle cerebral artery.Changes of CBF values of CTP and 3D pcASL after CEA were assessed.The correlation of ΔCBF based on CTP and3D pcASL was evaluated.Results(1) Compared with CBF values based on CTP before CEA,CBF values of both affected side and contralateral side increased after CEA.(2) Compared with preoperative CBF values based on 3D pcASL before CEA,postoperative CBF values of both affected side and contralateral side increased.(3) Correlation analysis indicated that the ΔCBF based on 3D pcASL was positive correlated with ΔCBF based on CTP.Conclusion The evaluation effect of 3D pcASL on cerebral blood perfusion before and after CEA is similar to that of CTP.It can quantitatively evaluate the hemodynamic changes before and after CEA.更多还原
