【摘要】 目的 探讨IVIM在椎体骨质疏松评价的应用价值。方法 纳入42例骨质疏松患者及38例健康志愿者,进行双能X线吸收法(DXA)、常规DWI和IVIM成像,测量椎体的骨矿物密度(BMD)、ADC、慢速扩散系数(D)、快速扩散系数(D~*)及灌注分数(f),比较健康志愿者及骨质疏松患者的ADC和IVIM参数,采用Spearman相关性分析ADC值、IVIM参数与骨密度值(T)的相关性。结果 健康志愿者的ADC值、D值、D~*值和f值分别为(0.50±0.08)×10~3mm~2/s、(0.45±0.09)×10~3mm~2/s、(14.42±3.01)×10~3 mm~2/s和(0.12±0.04)%,而骨质疏松患者的ADC值、D值、D~*值和f值分别为(0.53±0.08)×10~3 mm~2/s、(0.39±0.06)×10~3mm~2/s、(11.29±3.37)×10~3mm~2/s和(0.13±0.03)%。骨质疏松患者的D、D~*值显著低于健康志愿者(P<0.05),并和BMD值之间呈中度正相关,相关系数分别为0.590和0.702,而ADC值和f值无统计学差异,且和BMD无相关性。结论 骨质疏松患者的D、D~*值下降,BMD存在一定相关性,IVIM成像可作为DXA的有效补充,是一种无创评价骨质疏松的方法。

【Abstract】 Objective To investigate the diagnostic value of IVIM in the evaluation of vertebral osteoporosis.Methods A total of42 patients with osteoporosis and 38 healthy volunteers were enrolled in this study.All subjects were underwent dual-energy X-ray absorptiometry(DXA),conventional DWI and IVIM imaging examination.Bone mineral density(BMD),ADC,D,D~* and f of the vertebral body were measured,respectively.ADC value and IVIM parameters were compared between healthy volunteers and osteoporosis patients.Spearman correlation was used to analyze the correlation between ADC value,IVIM parameters and bone mineral density values(T).Results The ADC value,D value,D~* value and f value were(0.50±0.08)×10~3 mm~2/s,(0.45±0.09)×10~3 mm~2/s,(14.42±3.01) × 10 ~3 mm~2/s and(0.12±0.04)% for healthy volunteers,and(0.53±0.08)×10~3 mm~2/s,(0.39±0.06)×10~3 mm~2/s,(11.29±3.37)×10~3 mm~2/s and(0.1 3±0.03) % for osteoporosis patients,respectively.The D and D~*values of osteoporosis patients were significantly lower than that of healthy volunteers(P<0.05),and there was a moderate positive correlation with BMD values.The correlation coefficients were 0.590 and 0.702,respectively.The ADC and f values showed no statistical differences between osteoporosis patients and healthy volunteers,also demonstrated no correlation with BMD.Conclusion The D and D~* values are decreased in osteoporosis,and show moderate correlation with BMD.The IVIM imaging can be used as an effective supplement to DXA,providing a non-invasive method for evaluating osteoporosis.
