联合80kV管电压与迭代重建技术在肾动脉CT血管成像中的应用价值
Application of combining 80 kV tube voltage and iterative reconstruction in renal CT angiography
【摘要】 目的 探讨联合80 kV管电压与多模型迭代重建(ASIR-V)技术对肾动脉CTA辐射剂量、对比剂用量及图像质量的影响。方法 前瞻性选取拟行肾动脉CTA患者50例并随机分为A组和B组。A组采用120 kV管电压,600 mg I/kg对比剂用量,40%自适应统计迭代重建(ASIR)。B组采用80 kV管电压,350 mg I/kg对比剂用量,80%ASIR-V重建。分别测量右肾动脉、左肾动脉及同层面右侧竖脊肌CT值和标准差(SD)值,并计算肾动脉SNR值和CNR值。由2名具有5年以上工作经验的影像科医师对肾动脉图像质量采用5分法进行双盲法主观评分。结果 2组患者人口特征差异无显著统计学意义(P>0.05)。B组有效辐射剂量(2.07 mSv±0.21 mSv)和对比剂总量(21.10 g±2.86 g)较A组(6.69 mSv±1.83 mSv和38.50 g±4.14 g)显著降低约69.1%和45.2%(P<0.001)。2组肾动脉强化值(CT值)相似且无显著统计学差异(P>0.05),B组肾动脉SD值显著低于A组(P<0.001),SNR值和CNR值显著高于A组(P<0.001)。2名医师对肾动脉图像主观评分一致性极好(Kappa值>0.08,P<0.05),B组图像质量主观评分显著高于A组(P<0.001)。结论在肾动脉CTA中,联合80 kV和ASIR-V可以提供比常规120 kV更好的图像质量,同时显著降低辐射剂量和对比剂剂量。
【Abstract】 Objective To investigate the effects of using 80 kV tube voltage and adaptive statistical iterative reconstruction Veo(ASIR-V)algorithm in renal CTA radiation dose,contrast agent and image quality.Methods Fifty patients with renal CTA in our hospital were prospectively collected and randomly divided into group A and group B.Group A was scanned with 120 kV tube voltage,600 mg I/kg contrast agent and was reconstructed with 40% ASIR.Group B was scanned with 80 kV tube voltage,350 mg I/kg contrast agent and was reconstructed with 80%ASIR-V.The CT values and standard deviation(SD) values of right renal artery,left renal artery and right erector spinae at the same level were measured,and the SNR values and CNR values of renal arteries were calculated.The image quality of renal arteries was subjectively scored by two experienced radiologists blindly using a 5-point system.Results There was no significant difference in population characteristis between the two groups(P>0.05).The effective radiation dose(2.07 mSv±0.21 mSv) and the total contrast agent(21.10 g±2.86 g) in group B were significantly lower than those in group A(6.69 mSv+1.83 mSv and 38.50 g±4.14 g)by 69.1 % and 45.2%(P<0.001).The CT values of renal arteries between the two groups were similar and there was no significant difference in CT values(P>0.05).Compared with group A,the SD values of renal arteries in group B were significantly lower(P<0.001),and the SNR values and CNR values in group B were significantly higher(P<0.001).The subjective scores of renal CTA image quality by two radiologists had excellent agreement(Kappa value>0.08,P<0.05),and subjective scores in group B were significantly higher than those of group A(P <0.001).Conclusion The combination of 80 kV tube voltage and ASIR-V algorithm can provide better image quality with reducing radiation dose and reducing contrast agent in renal CTA compared with the conventional 120 kV.
