IDEAL-IQ定量评估非酒精性脂肪肝与冠状动脉钙化的相关性

【摘要】 目的 应用MRI定量非对称回波最小二乘估算法迭代水脂分离(IDEAL-IQ)技术定量评估非酒精性脂肪肝(NAFLD)与冠状动脉钙化(CAC)的相关性。方法 选取NAFLD患者47例及40例健康志愿者(对照组),2组均行常规肝脏超声、CT、上腹部MR常规及IDEAL-IQ序列扫描、冠状动脉MSCT扫描及血生化检查,NAFLD组中22例患者行细针穿刺活检术明确诊断。IDEAL-IQ计算脂肪含量(FF),冠状动脉CTA计算冠状动脉钙化积分(CACS),比较FF与CACS是否有相关性。结果 NAFLD组FF显著高于对照组(P<0.001);NAFLD组CACS显著高于对照组(P<0.001);NAFLD组FF与CACS有相关性,且呈正相关(R~2=0.556 1,P<0.01)。结论 临床工作中除对NAFLD患者进行针对性诊治,还应考虑到其发生CAC的风险,及时筛查患者的冠状动脉情况,这对冠心病的早期预防至关重要。

【Abstract】 Objective To investigate the correlation between nonalcoholic fatty liver disease(NAFLD) and coronary artery calcification(CAC) by iterative decomposition of water and fat with echo asymmetry and least-squares estimation quantitation sequence(IDEAL-IQ).Methods A total of 47 patients with NAFLD were enrolled as NAFLD group and 40 healthy volunteers as control group.Both groups underwent liver ultrasound,CT examination,upper abdominal MR routine scan and IDEAL-IQ sequence scan,coronary MSCT scan and blood biochemical examinations,and 22 patients in the NAFLD group were diagnosed by fine-needle aspiration biopsy.Fat fraction(FF) and coronary artery calcification score(CACS) were calculated by IDEAL-IQ and CTA respectively,to evaluate whether there was a correlation between FF and CACS.Results The FF of NAFLD group was significantly higher than that of the control group(P<0.001).The CACS of NAFLD group was significantly higher than that of the control group(P<0.001).FF was positively correlated with CACS in NAFLD group(R~2=0.556 1,P <0.01).Conclusion The risk of CAC should be considered when NAFLD patients are diagnosed and treated.Timely screening of the coronary artery for them is essential for early prevention of coronary heart disease.
