高分辨MR对颈动脉体瘤的临床应用价值
Clinical value of high-resolution MR in diagnosis of carotid body tumor
【摘要】 目的 探讨高分辨MR对颈动脉体瘤(CBT)的临床价值。方法 选取行高分辨MR检查并经病理证实的CBT患者36例。按照瘤体大小及对血管包绕程度进行分型,将MR分型与病理结果对照,并分析术中出血量与分型的关系;合并颈动脉斑块者,判定斑块的位置、数目及性质,并分析颈动脉斑块与CBT的关系。结果 MR检出CBT 42个,其中37个经病理证实,Ⅰ型23个(22例),Ⅱ型14个,MR分型与病理结果一致。Ⅱ型CBT术中出血量明显高于Ⅰ型,差异有统计学意义(P<0.05);共13支颈动脉合并斑块,检出斑块18个,其中10个(55.6%)发生于颈总动脉分叉处,5个(27.8%)发生于颈内动脉。钙化斑块1个,早期脂质斑块12个,脂质核5个;单因素与多因素Logistic回归分析结果显示颈动脉斑块的发生与CBT有关(OR=12.208,95%CI=1.423~104.740,P=0.023)。结论 高分辨MR能够准确行CBT分型及颈动脉斑块评价,CBT术中出血量与分型有关,CBT也可导致颈动脉斑块的发生率增加。
【Abstract】 Objective To investigate the clinical value of high-resolution MR in diagnosis of carotid body tumor(CBT).Methods A total of 36 cases with CBT confirmed by pathology underwent high-resolution MR examination were colected.According to the size of the tumor and the degree of vascular involvement,the MR classification was compared with the pathological results,and the relationship between intraoperative blood loss and classification were analyzed.As for the carotid plaque,the location,number and nature of plaque were determined,and the relationship between carotid plaque and CBT were analyzed.Results 42 CBT were found on MR and 37were confirmed by pathology,including 23 lesions of type Ⅰ(22 cases) and 14 of type Ⅱ,the MR classification was consistent with pathological results.The amount of bleeding in type Ⅱ CBT was significantly higher than that in type Ⅰ(P<0.05).A total of 13 carotid arteries were found with plaques,and 18 plaques were detected,10 of which(55.6 %) occurred in the common arterial bifurcation,and5(27.8 %) occurred in the internal carotid artery.1 calcified plaque,12 early lipid plaques and 5 lipid nucleus were also found.The results of univariate and multivariate Logistic regresssion analysis showed that the occurrence of carotid plaque was related to CBT(OR=12.208,95 % CI=1.423-104.740,P=0.023).Conclusion High-resolution MR can accurately predict CBT type and carotid plaque.The intraoperative blood loss in CBT is related to the types,and CBT can also increase the incidence of carotid plaque.
