【摘要】 目的 分析后肾腺瘤(MA)的CT表现，提高对MA的诊断准确率。方法 收集8例经病理证实的MA患者的影像及临床资料，分析肿瘤的位置、大小、形态、边界、密度、强化特点及其与周围组织关系，总结其影像表现。结果 8例肿瘤均为孤立性，4例肿瘤位于左肾，4例位于右肾。6例位于肾皮质呈外生性生长。实性肿瘤6例，囊实性2例。2例可见斑点状钙化。同肾实质相比，平扫时呈等、略高密度，平均CT值为(42.9±9.0) HU。增强扫描后8例肿瘤皮髓期轻中度强化，实质期持续强化，进行了排泄期扫描的6例患者中，4例呈持续性强化，2例强化幅度略下降，各期强化幅度均低于正常肾实质。结论 MA平扫呈等或略高密度，增强扫描轻中度强化，可伴有钙化，具有以上影像表现的，应该考虑到MA。MA影像表现无特异性，需要结合临床资料综合评价。更多还原

【Abstract】 Objective To analyze the CT manifestations of metanephric adenoma(MA),to improve the diagnostic accuracy.Methods The imaging and clinical data of 8 cases with MA confirmed by pathology were analyzed including the location, size, shape, boundary, density, enhancement type and the relationship with the surrounding tissue.Results Tumors were solitary(8/8),in which there were 6 cases of solid and 2 cases with cystic components.Calcification was found in 2 cases.Tumors were located in the left kidney(4 cases) and right kidney(4 cases).The lesions were located in the renal cortex and exhibited exophytic growth in 6 cases.The attenuation of metanephric adenoma were more or equal compared to the renal parenchyma on unenhanced CT,and the average CT value was(42.9±9.0) HU.Dynamic contrast-enhanced CT revealed that enhanced degrees of MA tumors in the renal cortex, renal parenchymal and pelvic phase were all lower than that of normal renal parenchyma.A mild or moderate enhancement in the renal cortex phase and an even higher enhancement in the renal parenchymal phase were observed in all of the cases. Progressive enhancement in the pelvic phase was found in 4 cases and a slight decreased enhancement in the pelvic phase in 2 cases.Conclusion MA prents iso-dense or hypo-dense on unenhanced CT and mild or moderate enhancement on ehhanced CT,which can be accompanied by calcification.With the above imaging findings, we should consider the possibility of MA.CT features of MA are non-specific, a comprehensive evaluation of the clinical data should be combined.更多还原
