【摘要】 目的:分析彩色多普勒超声诊断宫颈癌及其盆腔淋巴结转移的临床价值。方法:收集宫颈癌患者100例,术前均进行彩色多普勒超声检查。分析影像学表现,包括回声、血流信号、血流动力学参数、淋巴结大小及是否出现盆腔转移等情况。以手术病理结果作为参照,探讨彩色多普勒超声检查对宫颈癌盆腔淋巴结转移的诊断效能。结果:彩色多普勒超声对宫颈癌分期诊断正确率为85.00%,与组织病理学检查比较无统计学差异(P>0.05)。彩色多普勒超声检出盆腔淋巴结转移者51例,阳性检出率为79.68%。有无盆腔淋巴结转移患者血流特征、回声类型及血管阻力指数(RI)值比较存在统计学差异(P<0.001)。超声检查显示子宫出现不同程度的增大,宫颈形态不规则,宫颈线偏移或显示欠清,侵犯宫内可出现子宫内膜回声连续性中断等相应回声改变,51例发生盆腔淋巴结转移者RI值均低于0.4,血流信号较丰富。结论:彩色多普勒超声检查可作为评估宫颈癌术前临床分期的有效依据,可直观显示盆腔淋巴结转移图像特征,检出率高。更多还原

【Abstract】 Objective:To analyze the clinical value of color Doppler ultrasound in the diagnosis of cervical cancer and pelvic lymph node metastasis.Methods:100 patients with cervical cancer were collected,and all patients were examined by color Doppler ultrasound before operation.The imaging findings including echo,blood flow signal,hemodynamic parameters,lymph node size and pelvic metastasis were analyzed.The diagnostic value of color Doppler ultrasound in pelvic lymph node metastasis of cervical cancer was explored by using pathological results as reference.Results:The accuracy rate of color Doppler ultrasound in the staging of cervical cancer was 85.00%,and there was no significant difference compared with histopathology(P>0.05).Color Doppler ultrasound detected 51 cases of pelvic lymph node metastasis,the positive detection rate was 79.68%.There were significant differences in blood flow characteristics,echo type and RI value between patients with or without pelvic lymph node metastasis(P<0.001).Ultrasound examination showed that the uterus had different degrees of enlargement,irregular shape of the cervix,deviation or unclear display of the cervical line,and the corresponding echo changes such as continuous interruption of the endometrial echo could occur in the invasion of the uterus.The RI values of 51 patients with pelvic lymph node metastasis were all less than 0.4,and the blood flow signals were rich.Conclusion:Color Doppler ultrasound can be used as an effective basis in evaluating the preoperative clinical staging of cervical cancer,can directly display the image characteristics of pelvic lymph node metastasis,and has a high detection rate.更多还原
