【摘要】 目的探讨卵巢透明细胞肿瘤的临床、影像和病理特征,提高诊治水平。方法回顾性分析1例卵巢透明细胞肿瘤临床资料,总结临床、影像和病理学特点并复习国内外相关文献。结果患者为49岁女性,查体彩色多普勒超声发现盆腔肿物,直径约10 cm。核磁共振检查:子宫前上方囊实性肿物,大小12.2 cm×11.1 cm×9.4 cm。行全子宫双附件切除术,术后病理左侧卵巢透明细胞肿瘤,肿瘤呈囊性乳头状,部分呈交界性透明细胞瘤,局部恶变为透明细胞癌;免疫组织化学肿瘤细胞肝细胞核因子(HNF)-1β阳性,WT-1阴性,雌激素受体(ER)阴性;给予多西他赛+卡铂/奥沙利铂化疗。随访12个月,病情未复发,目前患者一般情况良好。结论卵巢囊性乳头状透明细胞肿瘤罕见,其临床表现缺乏特异性,影像学有特异性表现,免疫组织化学有助于诊断,及时行病理检查、提高对少见病的认识是减少误诊的关键。更多还原

【Abstract】 Objective To explore the clinical, imaging and pathological features of ovarian clear cell tumor and improve the diagnosis and treatment. Methods The clinical data of a case of ovarian clear cell tumor were analyzed retrospectively, and the clinical, imaging and pathological characteristics were summarized. Results The patient was a 49 year old female. The diameter of pelvic mass was more than 10 cm, and the size of cystic solid mass was 12.2 cm× 11.1 cm × 9.4 cm. After the operation, the clear cell tumor of the left ovary was observed. The tumor was cystic papillary, some of them were borderline clear cell tumor, and some of them were malignant to clear cell cancer. The tumor cells were HNF-1 β positive, WT-1 negative, ER negative. Docetaxel and carboplatin or oxaliplatin were given.After 12 months follow-up, the disease did not recur. Conclusion Ovarian cystic clear cell tumor with papilla is rare,itsclinical manifestations lack specificity but imaging features are specific,immunohistochemistry is helpful for its diagnosis, timely pathological examination and improve the understanding of this diseases are the key to reduce misdiagnosis.更多还原
