应用下唇双蒂口轮匝肌黏膜瓣修复上唇红唇缺损畸形

【摘要】 目的探讨下唇双蒂口轮匝肌黏膜瓣修复上唇红唇缺损畸形的临床效果。方法根据红唇缺损量及部位,设计下唇口轮匝肌黏膜瓣的长度和宽度,双侧蒂部切口适当向口角内延伸,红唇复合组织瓣转位修复红唇缺损。结果本组8例患者术后口轮匝肌黏膜瓣血运良好,随访6~12个月,修复红唇饱满对称,口角形态自然,张口不受限,术区瘢痕不明显,上唇外观改善良好,下唇形态良好,无与手术相关的并发症发生。结论采用下唇双蒂口轮匝肌黏膜瓣修复上唇红唇缺损畸形具有手术操作简单,安全性高,切口隐蔽,修复效果好等优点,值得临床推广应用。

【Abstract】 Objective To explore the clinical efficacy of lower lip bipedicle orbicularis oris musculomucosal flap in the repair of upper-lip vermilion defect and deformity. Methods According to the area and location of vermilion defect, the length and width of lower lip orbicularis oris musculomucosal flap was designed. The incision of bipedicle was properly extended into the angulus oris. The composite tissue flap was applied in the transposition repair of the upper-lip vermilion defect. Results Eight patients were followed-up from 6 to12 months, the blood supply of orbicularis oris musculomucosal flap was good after operation. The appearance of upper-lip vermilion was full and symmetric. The morphology of angulus oris was natural and mouth opening was not limited. The scar in operation area was not obvious. The appearance of upper lip was improved well and the morphology of lower lip was good. No surgical related complications occurred. Conclusion The application of lower lip bipedicle orbicularis oris musculomucosal flap in the repair of upper-lip vermilion defect had advantages of simple manipulation, high security, invisible incision and good efficacy. It should be considered for popularization and application in clinic.
