淋巴细胞主动免疫治疗对胚胎移植反复失败患者妊娠预后的影响

【摘要】 为研究淋巴细胞主动免疫治疗对体外受精(in vitro fertilization,IVF)-胚胎移植(embryo transfer,ET)反复失败患者的临床效果,将同意试行淋巴细胞主动免疫治疗的46对IVF-ET反复失败患者作为观察组,拒绝接受免疫治疗的32对IVF-ET反复失败患者作为对照组,应用ELISA检测2组患者治疗前后外周血抗HLA抗体的转阳情况,观察2组患者在IVF-ET过程中的妊娠相关情况。结果显示,观察组患者抗HLA抗体的转阳率为69.57%,观察组患者的胚胎种植率、临床妊娠率均明显高于对照组(P<0.05),而流产率明显低于对照组(P<0.05);2组患者的获卵数、受精卵母细胞数、优质胚胎数、移植胚胎数的差异均无统计学意义(P> 0.05)。提示淋巴细胞主动免疫治疗可提高患者抗HLA抗体水平,改善妊娠结局,可作为治疗IVF-ET反复失败的有效技术手段。

【Abstract】 We aimed to investigate the effect of active immunotherapy with lymphocytes on repeated implantation failure occurred in patients undergone in vitro fertilization(IVF)-embryo transfer(ET). Seventy-eight pairs of patients diagnosed as repeated implantation failure were selected and divided into two groups according to their treatment, the observation group(n=46) and the control group(n=32). Active immunotherapy with lymphocyte was performed in the observation group. The positive conversion rate of anti-HLA antibody was examined by ELISA and pregnancy outcome was compared between the two groups. The results showed that anti-HLA antibody positive conversion rate in observation group was increased by 69.57%. The clinical pregnancy rate and embryo implantation rate in the observation group were higher than thos in the control group(P < 0.05), while the miscarriage rate was lower than that in the control group(P < 0.05). Furthermore, there was no significant difference in the number of retrieved oocytes, the number of fertilized oocytes, the number of high quality embryos and the number of embryos transferred between the two groups(P > 0.05). Taken together, active immunotherapy with lymphocyte can improve the outcome of IVF-ET in patients with repeated implantation failure, and this technology applied pre-IVF-ET may be effective for repeated implantation failure.
