疫情下孕妇不同的应对方式、社会支持对心理健康状况的影响

【摘要】 目的了解突发公共卫生事件COVID-19疫情下,孕妇不同的应对方式和社会支持对心理健康状况的影响,为改善孕妇心理状态,制定缓解孕妇心理压力,提高妊娠质量,平安度过妊娠期的临床干预策略提供科学依据及参考。方法于2020年2月20日至26日,通过线上平台采取方便抽样方法随机抽取陕西省10个地市1 992名孕妇作为调查对象,采用非参数检验、相关分析方法分析孕妇的心理健康状况、应对方式和社会支持情况及其对心理健康的影响。结果 COVID-19疫情下不同孕期、学历、居住地、收入和胎次的孕妇积极应对方式得分比较,差异具有统计学意义(H值分别为7.69、124.27、68.29、66.13、11.05,均P<0.05),不同年龄和居住地的孕妇在消极应对方式上得分比较,差异具有统计学意义(H值分别为16.18、90.52,均P<0.05);不同孕期、学历、收入的孕妇社会支持得分比较,差异具有统计学意义(H值分别为7.63、25.76、11.79,均P<0.05)。在心理健康状况方面,孕晚期和孕中期得分显著低于孕早期(H=10.00,P<0.05),学历越高、收入越高、城市、第三胎及以上的孕妇心理健康状况越差(H值分别为14.12、30.02、10.44、17.29,均P<0.05);Spearman相关分析显示,积极应对方式得分、消极应对方式得分、社会支持总分与心理健康状况显著相关(r值分别为-0.09、0.15、-0.28,均P<0.01)。回归分析显示,社会支持(β=-0.24)、消极应对方式(β=0.29)、积极应对方式(β=-0.16),可以预测心理健康状况(F=119.08,P<0.001)。结论孕早期、高学历、城市、高收入的孕妇是疫情期间出现心理问题的高危人群,孕妇获得的社会支持、采取的应对策略是疫情期间影响心理健康状况的重要因素。

【Abstract】 Objective To understand the impact of different coping styles and social support on mental health status of pregnant women during the COVID-19 epidemic,so as to provide scientific basis and reference for improving the psychological state of pregnant women,formulating clinical intervention strategies to relieve psychological pressure of pregnant women,improve the quality of pregnancy and survive through pregnant period.Methods From February 20 to 26,2020,a convenient sampling method was used to randomly select 1 992 pregnant women from 10 prefectures and cities in Shaanxi Province as the survey object through online platforms.Nonparametric test and correlation analysis were used to analyze their mental health status,and the impact of coping styles and social support on mental health status of pregnant women.Results There were significantly differences in the scores of positive coping styles among pregnant women with different pregnant periods,education,place of residence,income and parity during the COVID-19 epidemic(H=7.69,124.27,68.29,66.13 and 11.05,respectively,all P<0.05).And there were statistically significant differences in the negative coping styles among pregnant women of different ages and places of residence(H=16.18 and 90.52,respectively,both P<0.05).The differences of social support scores among pregnant women with different pregnancy,education and income were statistically significant(H=7.63,25.76 and 11.79,respectively,all P<0.05).In terms of mental health status,scores in the third trimester and second trimester were significantly lower than those in the first trimester(H=10.0,P<0.05).Pregnant women with higher education,higher income,living in the cities and having the third child or above had poorer mental health status(H=14.12,30.02,10.44 and 17.29,respectively,all P<0.05).Spearman correlation analysis showed that the scores of positive coping style,negative coping style and social support were significantly correlated with mental health status(r=-0.09,0.15 and-0.28,respectively,all P<0.01).Regression analysis revealed that social support(β=-0.24),negative coping style(β=0.29) and positive coping style(β=-0.16) predicted mental health status(F=119.08,P=0.001).Conclusion Pregnant women in early pregnancy,and with high education,living in urban and high-income are the high risk groups for psychological problems during the pandemic.The social support received and the coping strategies adopted by pregnant women are important factors influencing mental health status during the pandemic.
