氨溴索和糖皮质激素对早产儿BPD的有效性分析
Therapeutic effect of ambroxol combined with glucocorticoid on bronchopulmonary dysplasia of premature infants and its impact on their serum levels of IL-1β and TNF-α
【摘要】 目的探讨氨溴索联合糖皮质激素对早产儿支气管肺发育不良(BPD)的有效性及血液中白介素(IL)-1β、肿瘤坏死因子(TNF)-α水平的影响。方法选取2017年1月至2018年12月潍坊市妇幼保健院收治的早产儿BPD患儿98例,按照数字表法将其随机分为对照组和研究组。对照组应用常规治疗方法(主要包括吸氧、营养支持、利尿、抗感染、补充肺泡表面活性物质等);研究组在对照组的基础上应用氨溴索联合糖皮质激素治疗方法,分析两组的治疗效果及对血液中IL-1β和TNF-α水平的影响。结果研究组治疗有效率(93.9%)显著高于对照组(75.5%),χ~2=4.241,P <0.05;研究组IL-1β[(34.4±5.7)pg/mL]显著低于对照组[(88.8±15.2)pg/mL],t=14.564,P <0.05;研究组TNF-α[(91.5±11.3)pg/mL]显著低于对照组[(176.2±25.6)pg/mL],t=15.667,P <0.05;研究组吸氧时间[(5.1±1.1)d]显著少于对照组[(7.5±2.4)d],t=14.267,P <0.05;研究组机械通气时间[(8.7±1.6)d]显著少于对照组[(13.5±3.5)d],t=15.674,P <0.05;研究组不良反应发生率(2.0%)显著低于对照组(6.1%),χ~2=4.077,P <0.05。结论对早产BPD患儿采用氨溴索联合糖皮质激素治疗可提高疗效,促进患儿机体恢复,临床上应当进一步推广应用。
【Abstract】 Objective To investigate therapeutic effect of ambroxol combined with glucocorticoid on bronchopulmonary dysplasia(BPD) of premature infants and impact of the therapy on their serum levels of IL-1β and TNF-α. Methods 98 premature infants with BPD who admitted to Weifang Municipal Maternal and Child Health Hospital from January, 2017 to December, 2018 were selected as study subject and were randomly divided into two groups-study group and control group in digital table method. The premature infants in the control group were treated with routine treatment including oxygen inhalation, nutrition support, diuresis, anti-infection, administration of pulmoalveolar surfactant and so on, while those in the study group were treated with ambroxol combined with glucocorticoid on the basis of the routine treatment above. The therapeutic effects of the premature infants in the two groups and impact of the therapy on the serum levels of IL-1 beta and TNF-alpha were analyzed. Results The effective rate in the study group was significantly higher(93.9%)than that in the control group(75.5%)(χ~2 = 4.241, P<0.05). After treatment, the serum level of IL-1β of the premature infants in the study group(34.4±5.7 pg/mL) was significantly lower than that of those premature infants in the control group(88.8±15.2 pg/mL)(t = 14.564, P<0.05), and the serum level of TNF-α of the premature infants in the study group(91.5±11.3 pg/mL) was also significantly lower than that in the control group(176.2±25.6 pg/mL)(t = 15.667, P<0.05). The oxygen inhalation time of the premature infants in the study group(5.1±1.1 d) was significantly shorter than that of those premature infants in the control group(7.5±2.4 d)(t = 14.267, P<0.05). The mechanical ventilation time in the study group(8.7±1.6 d) was also significantly shorter than that in the control group(13.5±3.5 d), there was a significant difference between the two groups(t = 15.674, P<0.05). The incidence of adverse reactions in the study group was 2.0%, and that in the control group was 12.2%, there was a significant difference between the two groups(χ~2=4.077, P<0.05). Conclusion For premature infants with BPD, the therapy of ambroxol combined with glucocorticoid can effectively improve the therapeutic effect, promote recovery of the infants’ body. Thereby, the combined therapy should be popularized and applied further.
