【摘要】 运用中医药大数据智能处理与知识服务系统,对2007年1月至2017年12月在广东省中医院总院及其分院的妇科门诊中诊断为"经行头痛"的病案资料的临床特征及用药规律进行探讨,并对基于数据挖掘得到的新方进行分析。结果共筛选得到935张处方及125味中药,其中用药频次在140次以上的中药共20味。经行头痛多发于30～40岁女性,发病高峰多在小满、芒种、夏至节气。药物归经以肝经、脾经、心经为主。关联规则分析结果显示,组合1(党参、川芎、甘草、茯苓、白术、当归、熟地黄、白芍)和组合2(夜交藤、桑寄生、钩藤、天麻)为治疗经行头痛常用药物;聚类分析结果显示治疗经行头痛主要采用活血化瘀、平肝熄风、滋阴安神、补益气血、行气止痛、疏肝柔肝等药物。结果提示经行头痛多虚实夹杂,肝脾心不调是经行头痛的主因,调理肝脾气血为治疗的关键。更多还原

【Abstract】 Based on the Data Intelligent Processing and Knowledge Service System for Traditional Chinese Medicine,the medical information of outpatients with menstrual headache in the general and branch hospitals of Guangdong Provincial Hospital of Traditional Chinese Medicine admitted from January of 2007 to December of2017 were collected and analyzed for the exploration of the clinical characteristics and medication rules of menstrual headache. And the new candidate prescriptions obtained by data mining were also analyzed. A total of935 prescriptions involving 125 herbs were obtained. There were 20 herbs with the medication frequency over 140 times. Menstrual headache usually occurs in the women aged 30-40 years old,and the headache attacks usually in the solar terms of Grain Full(8 th solar term),Grain in Ear(9 th solar term),and the Summer Solstice(10 th solar term). The medicated herbs mainly have the meridian tropism to liver meridian, spleen meridian, and heart meridian. The results of association rule analysis showed that the herbal combination 1(Radix Codonopsis,Rhizoma Chuanxiong, Radix Glycyrrhizae, Poria, Rhizoma Atractylodis Macrocephalae, Radix Angelicae Sinensis,Radix Rehmanniae Preparata,Radix Paeoniae Alba)and the herbal combination 2(Caulis et Folium Polygoni Multiflori,Herba Taxilli,Ramulus Uncariae cum Uncis,Rhizoma Gastrodiae)were the commonly-used herbs for the treatment of menstrual headache. The cluster analysis results showed that the commonly-used herbs for the treatment of menstrual headache were the herbs for activating blood to remove stasis,herbs for calming liver and suppressing wind,herbs for nourishing yin and calming the mind,herbs for replenishing qi and blood,herbs for activating qi and relieving pain, and herbs for soothing liver and softening liver. The results indicated that menstrual headache is characterized by the deficiency syndrome mingling with the excess syndrome, and the disorder of liver,spleen and heart contributes to its primary pathogenesis,so the therapeutic principle for the treatment of menstrual headache is to regulate liver and spleen as well as qi and blood.更多还原
