浮针治疗顽固性面瘫的筋膜学机理探讨

【摘要】 总结浮针治疗顽固性面瘫的常用进针点、操作方法和常规治疗流程,结合中医基础理论、解剖学尤其是筋膜学原理,探讨浮针治疗顽固性面瘫的可能作用机制。浮针进针点与面瘫传统取穴非常类似,操作手法通过肌肉再灌注改善了患病区域的血液供应,通过对局部浅筋膜的大幅度扫散引起细胞膜张力改变等生物学效应,较远端针刺点的生物学效应沿着皮下间隙和神经血管束之间的间隙向病灶传递,浮针通过严格的针尖朝向病所的操作配合肌肉再灌注,使这种传递更加精准;而较近端针刺点则通过刺激面部肌肉的筋膜,增加了瘫痪肌肉的活性最终起到增加肌力的目的。

【Abstract】 To summarize the common acupuncture points,operation therapy and conventional treatment flow of floating needle in treating intractable fascial paralysis,and then combination with the basic theories of traditional Chinese medicine and anatomy, paticularly in theory fasciology, to explore the possible mechanism. Floating needle points are very analogous to traditional acupoints for fascial paralysis. Operation therapy improves the blood supply of the paralysis area through muscle reperfusion. Large sweep of local superficial fascia causes a series of biological effects such as changes in cell membrane tension. The biological effects of the distal needling points induced by floating needle are transmitted to the lesion area along the gap between the subcutaneous space and between the neurovascular bundle. This transmit makes more accurate when the needling manipulation with pinpoint direction to the lesion area is combined with muscle reperfusion,and the proximal needling points can increase the activity of the paralyzing muscles to ultimately increase muscle strength by stimulating the facial muscles’ fascia.
