慢性肾脏病患者FGF23与PTH及血钙、血磷的关系
Relationship between FGF23 and PTH and serum calcium and phosphorus in patients with chronic kidney disease
【摘要】 目的探讨血清成纤维细胞生长因子(FGF-23)在慢性肾脏病(CKD)患者中与甲状旁腺激素(PTH)及血钙、血磷的关系。方法选取2018年12月—2019年12月本院收治的慢性肾脏病患者86例作为研究对象,依据肾小球滤过率(eGFR)分为三组,分别为A组(CKD1-2期)28例,B组(CKD3-4期)30例,C组(CKD5期)28例。另选同期健康体检人员30名作为对照组。分别测定各组患者的FGF23,全段血甲状旁腺激素(iPTH)、血肌酐(Scr)、血钙(Ca)、血磷(P),采用SPSS 22.0软件处理,F检验用于计量资料,并采用■表示,χ~2检验用于计数资料,Pearson相关分析用于两变量之间的关系,以P<0.05为检验标准,检验有无统计学意义。结果与D组比较,A组eGFR下降,FGF23升高(P<0.05),B、C组iPTH、P、FGF23升高,eGFR、Ca下降(P<0.05);与A组比较,B、C组eGFR、Ca下降(P<0.05),iPTH、FGF23、P升高(P<0.05);与B组比较,C组eGFR、Ca下降(P<0.05),iPTH、FGF23、P升高(P<0.05)。CKD患者中血清FGF23与iPTH(r=0.092,P=0.000)、血磷(r=0.827,P=0.000)呈正相关;与血钙(r=-0.722,P=0.000)呈负相关。结论 CKD患者普遍存在矿物质代谢紊乱。CKD患者血清FGF23、iPTH、P水平随eGFR下降而升高,血钙水平随eGFR下降而降低。PTH、P与FGF23呈正相关,Ca与FGF23呈负相关。
【Abstract】 Objective To investigate the relationship between serum fibroblast growth factor(fgf-23and parathyroid hormone(PTH),serum calcium and phosphorus in patients with chronic kidney disease(CKD).Methods A total of 86 patients with chronic kidney disease,those hospitalized in the first affiliated hospital of Jiamusi university were selected as study subjects.According to the glomerular filtration rate(e GFR),the patients were divided into 3 groups:28 patients in A group(CKD1-2),30 patients in B group(CKD3-4),and28 patients in C group(CKD5).30 cases of healthy physical examination personnel during the same period were selected as the control group(group D).FGF23,blood parathyroid hormone(i PTH)of all segment,serum creatinine(Scr),blood calcium(Ca),blood phosphorus(P).All the data were analyzed by SPSS22.0 software,measurement data was expressed as■and analyzed by F-test,categorical data was analyzed by chi square test,Pearson correlation analysis was used for the relationship between two variables.P value less than 0.05 was considered to have statistical significance.Results Compared with group D,e GFR decreased and FGF23increased in group A(P<0.05),i PTH,P,FGF23 increased and e GFR and Ca decreased in group B and C(P<0.05).Compared with group A,e GFR and Ca in group B and C decreased(P<0.05),while i PTH,FGF23 and P increased(P<0.05).Compared with group B,e GFR and Ca in group C decreased(P<0.05),while i PTH,FGF23 and P increased(P<0.05).Serum FGF23 in CKD patients was positively correlated with i PTH(r=0.092,P=0.000)and blood phosphorus(r=0.827,P=0.000).There was a negative correlation with serum calcium(r=-0.722,P=0.000).Conclusions Mineral metabolism disorder is common in CKD patients.Serum levels of FGF23,i PTH and P in CKD patients increased with the decrease of e GFR,and serum calcium levels decreased with the decrease of e GFR.PTH,P was positively correlated with FGF23 and Ca negatively correlated with FGF23.
