新型冠状病毒肺炎临床分型与影像特点分析
Clinical classification and CT imaging features of corona virus disease-2019
【摘要】 目的探讨新型冠状病毒肺炎(COVID-19)的临床分型和影像征象特点。方法回顾性分析53例经临床证实COVID-19患者的临床和影像征象资料,依据《COVID-19诊疗方案》(第七版试行)临床分型分组,比较普通型与轻型、重型、危重型的临床特征和实验室指标差异,并比较普通型与重型、危重型的影像特征。结果临床资料中COVID-19普通型患者的年龄小于危重型(t=76.15,P<0.05),呼吸困难发生率明显低于危重型,C反应蛋白增高异常高于轻型(P均<0.05);普通型患者的白细胞异常和淋巴细胞减低均高于轻型,但低于重型、危重型(P均<0.05)。普通型COVID-19患者更倾向于多肺叶累及(P<0.05);铺路石征征象多见重症、危重症,均高于普通型(P均<0.05),其他间接征象中胸腔积液多为危重症者(P<0.05),而淋巴结肿大在三型中少见(P>0.05)。结论在COVID-19的诊断中临床表现、实验室检查和CT表现具有一定特征性,诊断时应结合患者流行病学史、临床表现和影像特征才能够更为精准地诊断COVID-19。
【Abstract】 Objective To explore the clinical classification and imaging characteristics of corona virus disease-2019(COVID-19).MethodsThe data of clinical and imaging of 53 patients who confirmed COVID-19 were retrospectively analyzed.According to the clinical classification of the "diagnosis and treatment guideline of COVID-19"(7 th edition),the clinical characteristics and laboratory indicators among general group,mild group,severe group,and critical group were compared.ResultsThe general type of patients with COVID-19 were younger than patients with critical type(t=76.15,P<0.05),the incidence of dyspnea was significantly lower than that of patients with critical type,and C-reactive protein was abnormally higher than patients with mild type(P<0.05).The leukocyte abnormalities and lymphopenia were lower in general type patients than in mild type patients,but lower than those in severe type and critical type patients(P<0.05).General type of COVID-19 were more likely to have multilobe involvement(P<0.05);signs of paving stones were more common in patients with severe and critical types,which were higher than those of the patients with general type(P<0.05).In other indirect signs,pleural effusion was more common in patients with critical type(P<0.05),but lymphadenopathy was rare in patients with mild,severe,and critical types(P>0.05).ConclusionIn the diagnosis of COVID-19,the clinical characteristics,laboratory indexes and CT imaging have some characteristics.The diagnosis of covid-19 should be combined with the epidemiological history,clinical characteristics and imaging.
