【摘要】 目的探讨乳头状肾细胞癌(PRCC)的临床及病理特征。方法回顾分析我院1996年10月至2015年2月收治的83例PRCC的临床及病理资料。结果患者年龄33～76岁,平均55.13岁。所有病例行根治性肾切除、保留肾单位手术或减瘤术,其中嗜碱型49例(59.04%),其CK7阳性率为91.84%(45/49)、CD10阳性率为24.49%(12/49);嗜酸型34例(40.96%),其CK7、CD10阳性率均为50.00%。总体5年生存率为89.16%(74/83),多元回归分析显示肿瘤分型、Fuhrman分级、T分期及是否远处转移是影响患者预后的因素。结论 PRCC作为独立类型的肾细胞癌具有独特的病理、免疫组化及影像学特点。根治性肾切除及保留肾单位手术是目前治疗PRCC的主要手段,其恶性度低,生长缓慢,预后较好。更多还原

【Abstract】 Objective To study the clinical and pathological characteristics of papillary renal cell carcinoma(PRCC). Methods The clinical and pathological data of 83 cases of PRCC treated in our hospital from October 1996 to February 2015 were retrospectively analyzed. Results The age range of the patients was from 33 to 76(average 55.13)years old.Radical nephrectomy,nephron sparing surgery or cytoreductive surgery was performed in all cases,including 49 cases of basophilic type(59.04%)with CK7 positive rate of 91.84%(45/49),CD10 positive rate of 24.49%(12/49),34 cases of eosinophilic type(40.96%)with CK7,CD10 positive rate of 50.00%.Overall5-year survival rate was 89.16%(74/83).Multiple regression analysis showed that tumor type,Fuhrman classification,T stage and distant metastasis were prognostic factors. Conclusions PRCC,as an independent type of renal cell carcinoma,has unique pathological,immunohistochemical and imaging characteristics.Radical nephrectomy and nephron sparing surgery are the main treatment methods at present,which have low malignancy,slow growth and better prognosis.更多还原
