CD24和MMP-2及TK1在不同病理类型结直肠息肉恶变筛查中的价值
Value of CD24 and MMP-2 and TK1 in screening malignant changes of colorectal polyps of different pathological types
【摘要】 目的 :研究簇分化抗原24(CD24)、基质金属蛋白酶-2(MMP-2)及胸苷激酶1(TK1)在不同病理类型结直肠息肉恶变筛查中的价值。方法:选择2018年6月—2019年2月在我院接受治疗的120例息肉患者,其中炎性息肉、管状腺瘤、绒毛管状腺瘤、绒毛状腺瘤各30例,选择同期30例正常肠黏膜及30例结直肠癌患者,比较各组患者组织及血清中CD24、MMP-2、TK1的表达,分析CD24、MMP-2、TK1评估结直肠癌的敏感度及特异度。结果:随着患者病理类型序列、增生程度、息肉的大小及数量的递增,其CD24、MMP-2、TK1的阳性率逐渐增加,差异有统计学有意义(P<0.05);结直肠癌患者血清CD24、MMP-2、TK1水平明显高于结直肠腺瘤性息肉患者,且结直肠腺瘤性息肉患者血清水平明显高于正常黏膜血清水平,差异有统计学意义(P<0.05);CD24敏感度为86.72%、特异度为56.67%,MMP-2敏感度为76.76%、特异度为73.33%,TK1敏感度为63.37%、特异度为70.42%。结论:随着患者病理类型序列、增生程度、息肉的大小及数量的递增其CD24、MMP-2、TK1的阳性率逐渐增加,通过检测血清CD24、MMP-2、TK1水平预测结直肠癌具有重要的临床价值,有助于结直肠癌的早期诊断。
【Abstract】 Objective: To study the value of CD24, MMP-2 and TK1 in screening malignant changes of colorectal polyps of different pathological types. Methods: From June 2018 to February2019 for treatment of 120 cases of adenomatous polyp patients, including inflammatory polyp, tubular adenoma, fluffy tubular adenoma, villous adenoma, 30 cases each, at the same time choose 30 cases of normal intestinal mucosa and 30 cases of colorectal cancer patients, comparing each patient’s tissues and serum CD24, the expression of MMP-2, TK1, analysis of CD24, MMP-2, TK1 rate the sensitivity and specificity of colorectal cancer. Results: The positive rates of CD24, MMP-2 and TK1 increased gradually with the increase of pathological type sequence, hyperplasia degree, size and number of polyps, and the differences were statistically significant(P<0.05). Serum CD24, MMP-2 and TK1 levels in colorectal cancer patients were significantly higher than those in colorectal adenomatous polyp patients, and serum levels in colorectal adenomatous polyp patients were significantly higher than those in normal mucosa, with statistically significant differences(P<0.05). The sensitivity of CD24 was86.72%, specificity was 56.67%, sensitivity of mmp-2 was 76.76%, specificity was 73.33%, sensitivity of TK1 was 63.37%, specificity was 70.42%. Conclusion: The positive rates of CD24, MMP-2 and TK1 increased with the sequence of pathological types, the degree of hyperplasia, and the size and number of polyps. The detection of serum levels of CD24, MMP-2 and TK1 is of great clinical value in predicting colorectal cancer, which is conducive to the early diagnosis of colorectal cancer.
