【摘要】 目的探讨国产CalliSpheres载药微球化疗栓塞(DEB-TACE)联合热消融治疗原发性肝癌的临床疗效及其安全性。方法选取福建省肿瘤医院2016年1月—2019年7月收治的采用肝动脉化疗栓塞术(TACE)联合热消融治疗的患者77例,根据治疗方法分为传统肝动脉化疗栓塞术(cTACE)组54例,DEB-TACE组23例。cTACE组采用c TACE联合热消融治疗,DEB-TACE组采用DEB-TACE联合热消融治疗。评估2组TACE后热消融治疗前肿瘤控制情况,比较2组血液指标变化及治疗后不良反应和并发症发生情况,住院时间。结果 DEB-TACE组客观缓解率(ORR)高于cTACE组(P<0.05)。2组治疗前后白细胞计数(WBC)、血红蛋白(Hb)、血小板计数(PLT)、白蛋白(ALB)、总胆红素(TBIL)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、肌酐(Cr)、甲胎蛋白(AFP)比较,差异无统计学意义(P>0.05)。治疗前2组胆碱酯酶(CHE)比较,差异无统计学意义(P>0.05);治疗后DEB-TACE组CHE低于cTACE组(P<0.05)。2组不良反应及并发症发生率比较,差异无统计学意义(P>0.05)。2组住院时间比较,差异无统计学意义(P>0.05)。结论国产CalliSpheres DEB-TACE联合热消融治疗原发性肝癌的临床疗效确切,能更有效地灭活瘤灶,利于后续热消融治疗,且安全性较高。但远期效果仍需进一步随访观察,并有赖于多中心、大样本进一步研究。更多还原

【Abstract】 Objective To investigate the efficacy and safety of domestic CalliSpheres of drug-eluting microsphere combined with thermal ablation on primary liver neoplasms.Methods A total of 77 cases of patients who were treated with transcatheter arterial chemoembolization(TACE)combined with thermal ablation were selected from January 2016 to July 2019 in Fujian Cancer Hospital,which were divided into into conventional TACE group(54 cases)and DEB-TACE group(23 cases).Patients in cTACE group were treated with cTACE combined with thermal ablation,and DEB-TACE group were treated with DEB-TACE combined with thermal ablation.The tumor control after TACE and before thermal ablation were evaluated.The changes of blood indexes,the incidence rate of adverse reactions and complications,and the length of hospital stay were compared between the two groups.Results The objective response rate(ORR)in DEB-TACE group was higher than cTACE group(P<0.05).There was no significant difference in WBC,Hb,PLT,ALB,TBIL,ALT,AST,Cr,AFP between the two groups before and after treatment(P>0.05).There was no significant difference in CHE between the two groups before treatment(P>0.05);after treatment,CHE in DEB-TACE group was lower than cTACE group(P<0.05).There was no significant difference in the incidence of adverse reactions and complications between the two groups(P>0.05).There was no significant difference in hospitalization time between the two groups(P>0.05).Conclusion Domestic Callispheres DEBTACE combined with thermal ablation in the treatment of primary liver cancer has definite clinical efficacy,can more effectively inactivate tumor focus,and is conducive to follow-up thermal ablation treatment,and has high safety.However,the long-term effect still needs further follow-up observation,and depends on multi center,large sample further study.更多还原
