【摘要】 目的:探讨眼轮匝肌缩短术联合自体眼轮匝肌填充在老年性下睑内翻患者中的应用效果及对美观的影响。方法:选择2016年1月-2018年12月笔者医院老年性下睑内翻患者38例作为研究对象,随机数字表法分为观察组(n=19例,25眼)和对照组(n=19例,23眼)。对照组采用眼轮匝肌缩短术治疗,观察组在对照组基础上联合自体眼轮匝肌填充治疗,术后1个月对患者效果进行评估,术后对患者进行12个月随访,比较两组术后1个月近期疗效、美观评分、术后并发症及复发情况。结果:观察组患者术后1个月近期有效率为100.00%,高于对照组的86.96%,差异有统计学意义(P<0.05)。两组术后12个月的美观评分均高于术前,差异有统计学意义(P<0.05)。观察组术后12个月重睑幅度、有无成角畸形、睑裂闭合、睑缘弧度及总体美观评分均高于对照组,差异有统计学意义(P<0.05)。两组术后并发症发生率及术后1个月复发率比较差异无统计学意义(P>0.05);观察组术后6个月、12个月复发率均低于对照组,差异有统计学意义(P<0.05)。结论:眼轮匝肌缩短术联合自体眼轮匝肌填充用于老年性下睑内翻患者中能获得较高的近期疗效,能提高患者术后美观,降低术后复发率,不增加并发症发生率,值得推广应用。更多还原

【Abstract】 Objective To investigate the effect of orbicularis oculi muscle shortening combined with autogenous orbicularis oculi muscle filling in the elderly patients with lower eyelid entropion and its effect on the beauty.Methods From January 2016 to December 2018,38 elderly patients with entropion of the lower eyelid in our hospital were selected as the subjects.They were randomly divided into the control group (n=19,23 eyes) and the observation group (n=19,25 eyes).The control group was treated by shortening the orbicularis oculi muscle.The observation group was treated with autogenous orbicularis oculi muscle filling on the basis of the control group.The effect of the patients was evaluated one month after the operation.The patients were followed up 12 months after the operation.The short-term effect,aesthetic score,postoperative complications and recurrence rate of the two groups were compared.Results The short-term effective rate was 100.00% in the observation group,which was higher than that in the control group(86.96%),the difference was statistically significant (P<0.05).The aesthetic score in the two groups was higher than that before operation (P<0.05).The range of double eyelid,angular deformity,closure of blepharoplasty,curvature of eyelid margin and the overall aesthetic score in the observation group were higher than those in the control group (P<0.05).There was no significant difference in the incidence of postoperative complications and 1 month recurrence rate between the two groups (P>0.05).The recurrence rate of the observation group was lower than that of the control group at 6 months and 12 months after operation (P<0.05).Conclusion The combination of orbicularis oculi myotomy and autogenous orbicularis oculi myoplasty can achieve a high short-term effect in the elderly patients with entropion of the lower eyelid,improve the postoperative beauty,without increasing the incidence of complications,and reduce the postoperative recurrence rate,which is worth popularizing.更多还原
