急性脑梗死患者UA及hs-CRP与其不同血管部位的颈动脉粥样硬化程度的相关性研究
Correlation between UA and hs-CRP and carotid atherosclerosis in different vascular sites in patients with acute cerebral infarction
【摘要】 目的探讨并分析急性脑梗死患者血尿酸(UA)、超敏C反应蛋白(hs-CRP)与其不同血管部位的颈动脉粥样硬化程度的相关性。方法纳入2018年2月至2019年2月期间于本院诊治的急性脑梗死患者120例进行研究,以是否存在动脉粥样硬化为依据,分为A组动脉粥样硬化脑梗死、B组非动脉粥样硬化脑梗死,再依据颈动脉内中膜厚度(IMT)检查结果将A组患者分为A1组(颈动脉内中膜增厚)、A2组(斑块形成)、及A3组(颈动脉狭窄),比较各组患者的UA及hs-CRP检测水平,并分析二者与急性脑梗死患者颈动脉粥样硬化程度的相关性。结果左侧粥样硬化斑块发生率高于右侧;两侧粥样硬化斑块均于分叉处最常见,其次为CCA、ICA、VA;A组患者血清hs-CRP、UA水平比B组更高,A2组血清hs-CRP、UA水平高于A1组,A3组血清hs-CRP、UA水平均高于A1组及A2组,差异均具有统计学意义(P<0.05);相关性分析表明,脉粥样硬化形成与血清hs-CRP及UA水平均呈正相关(r=0.947,P=0.006、r=0.919,P=0.000)。结论不同血管部位的颈动脉粥样硬化发生率存在差异,分叉处最高;血清hs-CRP及UA与颈动脉粥样硬化程度呈正相关,可促进颈动脉粥样硬化的形成。
【Abstract】 Objective To investigate and analyze the correlation between serum uric acid(UA),high-sensitivity C-reactive protein(hs-CRP)and carotid atherosclerosis in different vascular sites in patients with acute cerebral infarction.Methods 120 patients with acute cerebral infarction who were treated in our hospital from February 2018 to February 2019 were enrolled in the study.According to the presence or absence of atherosclerosis,they were divided into group A atherosclerotic cerebral infarction and group B non-arterial.Atherosclerotic cerebral infarction,according to carotid intima-media thickness(IMT)examination results,group A patients were divided into A1 group(carotid intima thickening),A2 group(plaque formation),and A3 group(neck)Arterial stenosis),the UA and hs-CRP levels were compared between the groups,and the correlation between the two was compared with the degree of carotid atherosclerosis in patients with acute cerebral infarction.Results The incidence of left atherosclerotic plaque was higher than that of right;both sides of atherosclerotic plaque were most common at bifurcation,followed by CCA,ICA and VA;the serum hs-CRP and UA levels of group A were higher than those of group B,the serum hs-CRP and UA levels of group A2 were higher than those of group A1,and the serum hs-CRP and UA levels of group A3 were significantly higher than those of group A1 and group A2,with statistical significance(P<0.05).Sclerosis was positively correlated with serum hs-CRP and UA levels(r= 0.947,P=0.006,r=0.919,P=0.000).Conclusion The incidence of carotid atherosclerosis varies in different vascular sites,with the highest bifurcation.Serum hs-CRP and UA are positively correlated with the degree of carotid atherosclerosis,which can promote the formation of carotid atherosclerosis.
