老年腔隙性脑梗死患者血脂与脑微出血风险的相关性研究
Correlation between blood lipids and cerebral microbleeds in elderly lacunar infarction patients
【摘要】 目的探讨老年腔隙性脑梗死(LI)患者血脂水平与脑微出血(CMB)风险的相关性。方法收集北京市海淀医院神经内科住院治疗的确诊为LI的老年患者219例,根据头颅磁敏感加权成像(SWI)序列检查结果分为CMB组91例和不伴CMB组128例,回顾性分析患者临床资料,并用多因素logistic分析CMB风险的独立危险因素。结果与不伴CMB组比较,CMB组高血压(85.7%vs 68.0%)、吸烟(37.4%vs 18.0%)比例显著升高,TC[(3.84±0.09)mmol/L vs(4.21±0.09)mmol/L]、TG[(1.25±0.10)mmol/L vs(1.41±0.08)mmol/L]、LDL-C[(2.19±0.07)mmol/L vs(2.45±0.07)mmol/L]水平降低,差异有统计学意义(P<0.05,P<0.01)。多因素logistic分析显示,高血压(OR=0.287,95%CI:0.134~0.614),吸烟(OR=0.280,95%CI:0.141~0.554)和TC水平降低(OR=0.656,95%CI:0.472~0.911)是LI患者CMB风险的独立危险因素(P<0.05)。结论 TC水平降低可能是LI患者出现CMB的独立危险因素。
【Abstract】 Objective To study the correlation between blood lipids and cerebral microbleeds(CMB) in elderly lacunar infarction patients.Methods Two hundred and nineteen elderly lacunar infarction patients admitted to our hospital were divided into CMB group(n=91) and CMB-free group(n=128) according to their SWI.Their clinical data were retrospectively analyzed.The independent risk factors for CMB were analyzed by multivariate logistic regression analysis.ResultsThe incidence of hypertension was significantly higher,the history of smoking was significantly longer while the serum levels of TC,TG and LDL-C were significantly lower in CMB group than in CBM-free group(85.7% vs 68.0%,37.4% vs 18.0%,3.84±0.09 mmol/L vs 4.21±0.09 mmol/L、1.25±0.10 mmol/L vs 1.41±0.08 mmol/L、2.19±0.07 mmol/L vs 2.45±0.07 mmol/L,P<0.05,P<0.01).Multivariate logistic analysis showed that hypertension,smoking,low serum TC level were the independent risk factors for CMB in elderly lacunar infarction patients(OR=0.287,95%CI:0.134-0.614,OR=0.280,95%CI:0.141-0.554,OR=0.656,95%CI:0.472-0.911,P<0.05).Conclusion Low serum TC level is an independent risk factor for CMB in elderly lacunar infarction patients.
