幽门螺旋杆菌与冠心病及颈动脉硬化的相关性分析

【摘要】 目的 探讨幽门螺旋杆菌(Hp)与冠状动脉粥样硬化性心脏病(CHD)及颈动脉硬化的相关性。方法 选取2018年1月至2019年6月行冠脉造影术检查确诊为冠心病152例为观察组,非冠心病124例为对照组,比较两组患者Hp感染率。观察组根据Hp感染情况分为Hp阳性组和Hp阴性组,分析两组患者血脂、纤维蛋白原(FIB)、同型半胱氨酸(Hcy)、超敏C反应蛋白(hs-CRP)、总胆红素(TBIL)、胆红素血脂综合指数[LDL-C/(HDL-C+TBIL)及TC/(HDL-C+TBIL)]水平,超声检查是否存在颈动脉硬化。结果 观察组Hp感染率(55.9%)明显高于对照组(38.7%)(P<0.05)。观察组中Hp阳性组中胆固醇(TC)、低密度脂蛋白(LDL-C)、FIB、Hcy、hs-CRP、LDL-C/(HDL-C+TBIL)及TC/(HDL-C+TBIL)水平均明显高于Hp阴性组(P<0.05);高密度脂蛋白(HDL-C)、TBIL、间接胆红素(IBIL)水平低于Hp阴性组(P<0.05)。Hp阳性组不稳定斑块检出率(35.3%)大于Hp阴性组(16.4%),内中膜增厚检出率(8.2%)低于Hp阴性组(22.4%)(P<0.05)。结论 Hp感染可能引起脂质代谢紊乱、加剧炎症反应,影响冠心病的发展过程,增加颈动脉斑块不稳定。

【Abstract】 Objective To investigate the correlation between Helicobacter pylori(Hp) and coronary heart disease(CHD) and carotid atherosclerosis.Methods From January 2018 to June 2019,152 patients with coronary heart disease diagnosed by coronary angiography in our hospital were selected as observation group and 124 patients with non-coronary heart disease as control group.The Hp infection rates of the two groups were compared.The observation group was divided into Hp-positive group and Hp-negative group according to the infection of Hp.The effects of Hp on blood lipid,fibrinogen(FIB),homocysteine(Hcy),hypersensitive C-reactive protein(hs-CRP),total bilirubin(TRIL),bilirubin blood lipid index(LDL-C/(HDL-C+TBIL) and TC/(HDL-C+TBIL) levels were analyzed,and whether carotid atherosclerosis existed by ultrasonography.Results The infection rate of Hp in the observation group(55.9%) was significantly higher than that in the control group(38.7%)(P<0.05).The levels of cholesterol(TC),low density lipoprotein(LDL-C),FIB,Hcy,hs-CRP,LDL-C/(HDL-C+TBIL) and TC/(HDL-C+TBIL) in the Hp positive group were significantly higher than those in the Hp negative group(P<0.05),while the levels of high density lipoprotein(HDL-C),TBIL and indirect bilirubin(IBIL) were lower than those in the Hp negative group(P<0.05).The detection rate of unstable plaque in Hp positive group(35.3%) was higher than that in Hp negative group(16.4%).The detection rate of intima-media thickening(8.2%) was lower than that in Hp negative group(22.4%)(P<0.05).Conclusion Hp infection may cause lipid metabolism disorder,aggravate inflammation,affect the development of coronary heart disease,and increase the instability of carotid plaque.
