三七对接受PFNA治疗的老年股骨粗隆间骨折患者围术期隐性失血的影响

【摘要】 目的 观察三七对接受股骨近端防旋髓内钉(PFNA)治疗的老年股骨粗隆间骨折患者围术期隐性失血的影响。方法 2017年1月至2019年1月期间接受PFNA治疗的老年股骨粗隆间骨折患者69例,依据是否给予三七治疗分为两组,对照组36例未给予三七治疗,观察组33例给予三七治疗。测定血红蛋白与红细胞比容,统计隐性失血量、隐性失血百分比、高隐性失血发生率,同时对Harris评分、术后并发症进行比较。结果 术后两组血红蛋白与红细胞比容较术前明显下降(P<0.05),且观察组术后血红蛋白与红细胞比容明显大于对照组(P<0.05);观察组隐性失血量、隐性失血百分比、高隐性失血发生率均低于对照组(P<0.05);两组术后3个月Harris评分较术后1个月Harris评分明显增大(P<0.05),且观察组术后3个月Harris评分明显高于对照组(P<0.05);观察组术后并发症发生率明显低于对照组(P<0.05)。结论 接受PFNA治疗的老年股骨粗隆间骨折患者辅以三七治疗,有助于合理调节血红蛋白与红细胞比容,降低隐性失血量、隐性失血百分比、高隐性失血发生率,提高Harris评分,降低术后并发症发生率。更多还原

【Abstract】 Objective To observe the effect of pseudo-ginseng on the incidence of occult blood loss during perioperative period in elderly patients with intertrochanteric fracture treated with PFNA.Methods A total of 69 elderly patients with femoral intertrochanteric fracture treated with PFNA in our hospital from January 2017 to January 2019 were selected as the subjects and divided into two groups according to whether or not pseudoginseng was given,36 cases in the control group were not given pseudo-ginseng treatment,and 33 cases in the study group were given pseudo-ginseng treatment.The hemoglobin and hematocrit were measured,the occult blood loss,the percentage of occult blood loss and the incidence of high occult blood loss were counted,and Harris score and postoperative complications were compared.Results The hemoglobin and hematocrit in the two groups decreased significantly after operation(P<0.05),and the hemoglobin and hematocrit in the study group were significantly higher than those in the control group(P<0.05).The occult blood loss,the percentage of occult blood loss and the incidence of high occult blood loss in the study group were lower than those in the control group(P<0.05).The Harris score of the two groups at 3 months after operation was significantly higher than that of the control group at 1 month after operation(P<0.05),and the Harris score of the study group was significantly higher than that of the control group at 3 months after operation(P<0.05).The incidence of postoperative complications in the study group was significantly lower than that in the control group(P<0.05).Conclusions The treatment of pseudo-ginseng combined with PFNA in elderly patients with intertrochanteric fracture of femur is helpful to adjust hemoglobin and hematocrit reasonably,reduce the occult blood loss,the percentage of occult blood loss,the incidence of high occult blood loss,improve Harris score and reduce the incidence of postoperative complications.更多还原
