脊柱外科免输异体血技术在临床手术中的应用

【摘要】 目的:应用综合血液保护技术,通过对临床病例研究,制定安全可行的免输异体血措施和技术,保证手术患者的安全。方法:选取2017-01—2019-04在脊柱外科住院行择期手术治疗的患者135例,其中男69例,女66例;年龄12~85岁;应用综合免输异体血技术完成手术,对患者数据进行分析。结果:135例患者应用综合免输异体血技术顺利完成手术,术后恢复良好。血红蛋白(Hb)指标术后降低(P<0.05);术前、术后凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)差异无统计学意义(P>0.05);术后纤维蛋白(原)降解产物(FDP)较明显升高(P<0.05);术后血钾(K~+)有所降低(P<0.05),但均在正常值范围(3.5~4.5 mmol/L);术后超敏C-反应蛋白(hsCRP)水平显著升高(P<0.05)。结论:通过广泛的临床病例研究,制定免输异体血技术的措施,可以保证患者的安全,减少患者手术用血,值得临床应用和推广。

【Abstract】 Objective:To apply the comprehensive blood protection technology,through the study of clinical cases,established the safe,reliable and feasible measures and techniques fornon transfusion allogeneic blood,ensured the safety of surgical patients. Method:From January 2017 to April 2019,135 patients,69 males and 66 females,aged from 12 to 85 years,were selected for elective spine surgery in our hospital.The operation was completed by using non transfusion allogeneic blood technology,and the data of patients were analyzed. Result:135 patients successfully completed the operation with the technology of comprehensive non transfusion allogeneic blood technology,and recovered well after the operation.Hemoglobin(HB)decreased after operation(P<0.05),prothrombin time(PT)and activated partial thromboplastin time(APTT)had no significant difference before and after operation(P>0.05),fibrinogen degradation product(FDP)increased significantly after operation(P<0.05),potassium(K~+)decreased after operation(P<0.05),but they were all in normal range(3.5-4.5 mmol/L).The level of hsCRP increased significantly(P<0.05).Conclusion:Through a wide range of clinical case studies,the establishment of non transfusion allogeneic blood technology could ensure the safety of patients,reduce the blood use of patients,which is worthy of clinical application and promotion.
