抗人球蛋白IgG和C3d对不规则抗体筛查项目诊断价值分析
Diagnostic value of anti-human globulin IgG and C3d for irregular antibody screening
【摘要】 目的:分析输血相容性检测不规则抗体筛查项目使用的抗人球蛋白对输血相关特异性抗体的影响。方法:选取2016-05—2018-12待输血患者标本10 014例,应用多特异性(抗-IgG、抗-C3d)及单特异性(抗-IgG)抗人球蛋白进行不规则抗体检测及鉴定,并对阳性样本应用多特异性抗人球蛋白、单特异性抗-IgG、单特异性抗-C3d等不同递质的试管法比较验证。结果:多特异性抗人球蛋白检测抗体阳性共35例(阳性率0.35%),主要为Rh系统抗体和自身抗体,对35例阳性样本使用单特异性(抗-IgG)复查,其中27例复查结果与多特异性抗人球蛋白一致,6例与多特异性抗人球蛋白比较仅反应强度存在差异(强度高低者持平,差异均未超1+),2例单特异性(抗-IgG)复查为阴性,鉴定为样本在多特异性抗人球蛋白检测时血清中补体C3d成分被激活所致。结论:不规则抗体筛查选择多特异性抗人球蛋白可防止补体依赖型抗体漏检,选择单特异性抗-IgG则可避免临床不相关冷抗体引起的假阳性反应。单特异性抗-IgG检测不规则抗体,在提供了可靠性的同时避免了一些常见的干扰和检出无关紧要的结合补体类IgM抗体,从而节省时间和费用。
【Abstract】 Objective:To analyze the effect of anti-human globulin used in irregular antibody screening for transfusion compatibility testing on specific antibodies related to blood transfusion. Method:A total of 10 014 blood transfusion patients were selected from May 2016 to December 2018.The irregular antibodies were detected and identified by multi-specificity(anti-IgG,anti-C3d)and monospecific(anti-IgG)anti-human globulin.The positive samples were compared and validated by tube test with different media such as multi-specificity anti-human globulin,monospecific anti-IgG and monospecific anti-C3d. Result:A total of 35 patients(positive rate 0.35%)were detected by multi-specific anti-human globulin,mainly Rh blood group system antibodies and autoantibodies.35 patients of positive samples using monospecific review(anti-IgG),27 patients review results were consistent with more specific anti-human globulin,6 patients compared with more specific anti-human globulin reaction intensity differences only(flat of intensity,differences were not exceed 1 +),2 patients of monospecific(anti-IgG)were negative review.It was identified that the C3d component of complement in serum was activated when multi-specific anti-human globulin was detected.Conclusion:The purpose of selecting multi-specific anti-human globulin is to prevent the omission of complement dependent antibody.Selection of monospecific anti-IgG can avoid false positive reaction caused by clinically unrelated cold antibody.Monospecific anti-IgG to detect irregular antibodies can provide reliability while avoiding some common interference and detecting irrelevant binding complement IgM antibodies,thus saving time and cost.
