非侵入式眼表综合分析仪评估近视患者干眼和睑板腺功能障碍
Evaluation of Dry Eye and Meibomian Gland Dysfunction in Myopic Patients by Noninvasive Ocular Surface Analyzer
【摘要】 目的探讨非侵入式眼表综合分析仪评估近视患者干眼和睑板腺功能障碍(MGD)的价值。方法回顾性分析2017年6月至2019年12月住院或门诊就诊的120例(240眼)近视患者的临床资料。根据眼表疾病指数(OSDI)评分将患者分为干眼组(n=70)和正常对照组(对照组,n=50)。受检者均进行OSDI问卷调查、裂隙灯下行眼前节检查、Keratograph眼表综合分析仪查看眼部情况。比较2组眼表和睑板腺状况,分析干眼和眼表状况。结果与对照组比较,干眼组MGD和OSDI评分、角膜荧光染色评分均较高(P<0.05),泪河高度、非侵入性泪膜破裂时间(NIBUT)均较低(P<0.05)。干眼患者OSDI评分与NIBUT呈负相关(r=-0.392,P=0.001),与泪河高度无关(r=-0.203,P=0.091),与睑板腺缺失呈正相关(r=0.410,P<0.001)。结论近视患者中干眼的患病率较高,干眼人群存在MGD,非侵入式眼表综合分析仪检测干眼及MGD较为可靠,可提高干眼的检出率。
【Abstract】 Objective To investigate the value of noninvasive ocular surface analyzer in evaluating dry eye and meibomian gland dysfunction(MGD) in myopic patients.Methods The clinical data of 120 myopic patients(240 eyes) hospitalized in our hospital or treated at our clinic from June 2017 to December 2019 were analyzed retrospectively.According to the Ocular Surface Disease Index(OSDI) scores,these patients were divided into dry eye group(n=70) and normal control group(control group,n=50).OSDI questionnaire,slit lamp anterior segment examination and Keratgraph ocular surface analyzer were used to check the eye condition.The ocular surface and meibomian gland alterations were compared between the two groups to analyze the condition of dry eye and ocular surface in patients with myopia.Results Compared with control group,patients in dry eye group had MGD score with an increase in OSDI score and corneal fluorescence staining score and a decrease in tear meniscus height and non-invasive tear film break-up time(NIBUT)(P<0.05).The OSDI score in dry eye patients was negatively correlated with NIBUT(r=-0.392,P=0.001),but positively related to meibomian gland loss(r=0.410,P<0.001).It had no association with tear meniscus height(r=-0.203,P=0.091).Conclusion The prevalence of dry eye is higher in myopic patients,and MGD exists in dry eye population.Noninvasive ocular surface analyzer is reliable to detect dry eye and MGD and can improve the detection rate of dry eye.
